everyone welcome to community wealth uh what i'm going to do today is with this unemployment deo i'm going to just show you how to upload the documentation needed to prove income for well to continue receiving your unemployment benefits right so let's get to it on the home page you're going to click here you should have this up or you go through your message but you should have this okay so i'm going to go here okay so here we go on this page here on this page here it says acceptable proof of employment documentation could be paycheck stubs earnings leave statements showing the employer's name and address there'll be two forms when available sites of acceptable documentation for proof of self-employment may include state or federal employer identification numbers business licenses tax returns business receipts signed affidavits from persons verifying your self-employment business or occupational licenses 1099s or current bank statements with business name and address and here is the proof of plan start date of employment it could be letters offering employment statements affidavits by individuals with name and contact information verifying and offer unemployment types of acceptable documentation for plan start date of self-employment may include business licenses states or federal employee notification numbers written business plans or a lease agreement all right so then we go down here where it says documentation to prove employment or self-employment needs to demonstrate the existence of employment or self-employment during the start of the applicable tax year and the dates of filing for pdua so it goes if your claim effective date is for the year 2020 then your applicable tax year is 2019. that would be me if your claim effective date is for the year 2021 then your applicable tax year is 2020. so that mean if you guys started to get your uh if you first started your claim date for unemployment this year then you have to use information from last year 2020 all right but for people like myself who filed from right when the pandemic started up until they just say possibly the middle of december then you can use 2019's information all right so let's go go with this here all right so with uh the 1099 situation and i'll have to read this several times because it says 10.99 or whatever the proof is and so i have proof all right i don't believe this is a 1099 it's called a tax summary right so i'm going to put it up here so with here here's the tax summary it says 2019 where we are alright says 2019 here it is this is for uber all right and i have my drive totals you know my net payout and all of that expenses fees and tax broke payment total net payout this is what's from right then i have lift all right this is lift right here all right this is attack summary now says your driving totals 516 10 305 miles here's the income and all that good stuff so i guess this is fine because it doesn't necessarily have to be a 1099 you just be proof and here's the proof right here all right this is proof so we're gonna use this to see what they say you know it's not like i don't have the information here it is right here has all the and that's pretty much it and as you can see i received another payment on the 19th all right so when i received the payment on the 19th also received a payment of the of the fpuc right so let's go look at that really quick look at that so right here we're going to look at the uh payment history i'm going to click here and as you can see on the 19th i received 226 on the fifth i received 226 the 22nd i received school 26 and the 20th i received okay so here i received one whatever okay so i received one two three four four payments all right this was for the one week it's for the week of december 27th and these are the subsequent bi-weekly payments since then all right so i received four payments total all right and same thing's gonna be for the fpuc so we're gonna go there all right so we're going to scroll down here you'll see right here all right here are the payments all right so we have this is from i want to say no that's not it okay so this would be from i want to say the week of december so that's that's what this one's gonna be something say that all right it's from the week of december and then these are the bad weekly payments after that all right so this is from the week of december the last week of december 270 and this is the bi-weekly payment since then all right so it's three of those all right so they're still paying so i received four total payments for both pua and fpuc so yes they're still paying guys they're still paying all right so all you have to do is go in here and you know take care of your business all right so again thanks for watching uh
