sometimes you're not going to find an exact match and that's okay so in my example I'm a receptionist it's under office and administrative support occupations and the category will do secretaries and a minister assistant sub-category it just gives us one option click on that and the type so it's either legal medical or any other except for legal and medical so we'll go ahead and choose that one there and then we'll hit select job classification like I said it doesn't have to be exact so they just want an idea of what we did start dates and end dates these need to be exact if you've worked there over two years give us your best guess for start date but end date does need to be exact okay supervisors name this is the last supervisor that you had and their telephone number you don't have their personal number just the business phone number is fine it doesn't like like how I entered this so I did put in the area code and spaces if it doesn't like how you entered something it'll tell you in red so no spaces or dashes and the phone numbers it looks like gross earnings if you've been there over a year just give us about your annual salary if you've only been there a few months look at your last pay stub because we will need a more exact amount in this case I earned thirty thousand a year so I'm just gonna enter thirty thousand we do have to enter a decimal no commas rate of pay sixteen dollars an hour and it will let you choose rate of pay you can choose if you were paid hourly daily weekly bimonthly or even yearly in this case if I was paid thirty thousand a year I could say I was paid sixteen dollars an hour or I could even put in here that it was thirty thousand dollars yearly so we'll just go ahead and stick with the sixteen dollars an hour is it a staffing agency no and are you scheduled to return to work for this employer if you say yes it'll ask you for your return date and then it'll ask you the reason that you're no longer working there and it does have correct that the dates are exact ok so I made seven dollars an hour what I would do this is very recent work history I would look at my most recent pay stub to find out exactly how much I had made year to date ok and in this instance I would say that I was laid off due to lack of work they a lot of the people started leaving Arizona because the weather was very hot they thought they would need another barista but they had over staffed so they just didn't have any additional work for me to do ok so I've entered all of my work history here to go back to the claimant application I simply go up to the ribbon here under apply for benefits and just click here and he'll take me back to the main page now you can see that the wages and work history are complete
