employment a copy of your dd214 if you had federal employment your copy of the sf-8 or sf-50 if available this is more information about your privacy and security identity identity verification program once you have read all this you can start filing a claim right over here and again finally climb home let's get started click continue a question all questions marked with an asterisk are blank and click on i do not have a pen the information that you enter in the beginning like your social security number date of claim your birthday your first name your last name uh gender what is going to be taken for the questionnaire that you answered now here say please select applicable form or of identification id type new jersey driver's license issue id if you say none then you have to can contact you you can enter here enter the number up here that is your main number the number that cell phone number whatever that you always answer uh you are a citizen or national united states yes or no um at your schedule for an appointment if you are scheduled for an appointment you need an interpreter if you need an interpreter you select here the language that you speak if the language that you speak other than english of course is not listed then you can type it here select the number of years education that you have completed here in this little arrow once you click there it's going to be a list of answers you answer the one that applies to you that says the number of years of education that you have then you have here what is your ethnic group you click again this little arrow there is a drop down drop down menu you can select the one that applies to you and then what is your race same thing you click here and select when you are working are you the primary main wage and earner and your household yes or no and then continue for this exercise we type the occupation as a warehouse and what is your job description you type it in here and then once you click the search it will bring you here so that's what you type in and then in the occupation you select which one of these choices from the unemployment insurance application applies to you so for this exercise we selected the first one once you select your occupation the script the best description here you click continue and again everything with the asterisk needs to be answered are you ready willing and able to immediately work full time of course you are you need a job so yes are you have you been a student of full-time attendant and educational institution within the last 18 months no are you currently enrolled in job training or college now if yes you have to say here what it is and the when the training started you can click here in this calendar and it will give you a choice that you can select if you want the ten percent of uh your federal income tax with health from your benefits yes or no do you wish to claim a dependency allowance yes that you have uh someone that you care care for and have to be responsible for that person then you say yes are you a member of a union yes or no if you are then it's going to tell you which union group is selected here you select on this drop down and it will give you a menu in which you can select for this exercise we select direct deposit now once you select direct deposit it's going to ask you for the account type you click here and that's little arrows will be a checking account savings account you need a routing number you enter the routing number here again the routing number and your checking account will be the first set of numbers and the left side of your check you can see that number right there then you enter your account number which under check or would be the number in the middle the long number here in the middle and then you enter that number here and here to verified and then when everything is good you click continue new jersey employer or employers assets more than one so we click continue if this is the place where you worked uh how you work for the employer listed above yes click continue then you're going to enter their address if it's not already listed you enter the address the phone brother i mean some kind of relation family business or whatever so in this case no nobody is related to the institution uh please select the reason for separation for this employer so here in this drop down menu you click on it and then you'll see the choices so for this exercise we selected laid off lack of work and then the uspec to be recalled by this employer so yes or no and in the case of covet 19 you know many uh places that are reopening are calling back their employees so you have to keep in mind if this is something that is going to be happening with you so you click continue and it's gonna give you the list of employers that you have and again if you have more than ne job it's gonna set here the employee name work and then if you missed an employer you can add another employer right here or if you were part of the military be besides having this this job this employer you can add a military employer if you only had one job like in this case so we can just go and click continue and here they're going to ask you the to enter the gross earrings earnings you have received from the city of paterson so your gross earning is the amount of money that you get before taxes after taxes would be your net before tax taxes is your growth so for this we put four thousand and then we click continue to hear since your last day worked have you collected this ability of workers compensation in this case no where you disable immediately before filing this unemployment insurance claim no so therefore the rest doesn't need to be answered so we click continue [Applause] going to submit your application but before you do that you're going to review it make sure that all the information is correct especially your email address and account numbers so without your social
