permanently and do not intend to reopen or restart doing your self-employment work you're unable to reach your place of employment because you have been advised by a public by a health care provider you're self-quarantined due to concerns related to covenanting so this would be individuals who have been advised they're in a high risk group category or if you have specific circumstances and your own medical provider has advised you to self quarantine then you would answer yes you would give a brief description it does not need to be detailed as far as why you're in a high-risk category or any of that and then the date that that began if we do have questions or need more information we'll reach out to you you're unable to reach your place of implant because of a quarantine imposed as a direct result of cover 19. um this was a common one due to this stay home stay safe so if uh if you have a situation like that or did have a situation like that or you live in another area and that continues to exist for you then you would mark yes and put the reason and the date that that started the next two questions help us to determine further eligibility for pandemic unemployment
