This is what it the default configuration looks like: The background image can be changed and you can add your own logo. Any text can be changed. Each question is asked on a separate screen so patients won’t miss anything. They can only progress to the next screen once they’ve filled in the current screen. Once the form has been filled out you can produce a PDF and store that in your patient management system. It’s super-configurable: ask any question, in any order – virtually any type of form can be made . You can get their photo and their signature. You can only ask if they could be pregnant if they’re female, rather than always like paper forms. Front-office iPads are meant for reception and, on launch of the app, load up the form you’ve configured it to without any intervention (e.g. medical history).
