Unfortunately, I can no longer post the answer to your question in one of my "Ask Me Anything" threads because I have been banned, and my previous explanation will no longer apply either. The short version is simply: If you're going to use an online sign-up tool, download the sign-up form and use it to create your own pdf document. After the form's done, simply copy and paste the code and click submit!
If you're going to use a desktop sign-up form, you don't need to create a pdf form because the pdf link will be automatically added to the form.
