hello and welcome to this tutorial on how to complete the tenant application for the louisiana emergency rental assistance program in this video i'll explain the purpose of each form and show you how to properly complete the required program documents let's get started you are preliminarily eligible to receive emergency rental assistance if the louisiana housing corporation emailed or mailed you the tenant required documentation packet this packet contains all of the documents that the tenants are required to complete there are three ways to submit your completed packet you can do that by email mail or fax which is indicated on this cover sheet please note that emailing your documents will expedite your application process now let's review the next form we have included a checklist to help you keep track of the program documents this checklist denotes who is responsible for completing the document it's recommended that you check the boxes assigned for the tenants to ensure that you complete and submit the required documentation there are three additional documents in this packet that require your signature and the landlord signature it's the request for unit approval the lease and lease addendum if you are unable to obtain your landlord signature your rental assistance specialist will attempt to obtain the signature on your behalf please note that not including the proper documentation could cause a delay in processing your application now let's review the first form the application is used to capture details about your living situation this information will be used to determine whether you're eligible to receive emergency rental assistance be sure to complete all fields as missing data could cause a delay in processing your application now let's review each section of this form in section 1 you are required to provide your contact information such as your name where you live email address and phone number you will also need to provide background information that includes your age date of birth race ethnicity and veteran status the case management section below will be completed by your rental assistance specialist in the event that we are unable to reach you please provide two emergency contacts in this area in section 2 you will provide information about the makeup of your household including the number of people that live with you along with their name social security number date of birth age sex and relationship to the head of household please note that not including this information may cause a delay in processing your application in section 3 you will provide information about your current living situation check the box that most accurately describes where you live if you're renting check other and specify whether you are renting an apartment or a house in the box below please describe your living situation this next area is voluntary and will not impact your overall eligibility check the boxes to indicate whether you or someone in your household is homeless elderly or disabled also indicate if you or anyone in your household requires accessibility due to vision hearing or physical impairment then explain the situation in the box below in section 4 you will provide information regarding your income you are only required to list your employment income or regular unemployment benefits do not include the enhanced unemployment benefits related to the coronavirus such as the additional six hundred dollars or three hundred dollar weekly payments in the employment information section include your employer's contact information and how much you are currently being paid if you are employed and your pay or work hours have been reduced please indicate the reduced amount and how often you're paid if you are unemployed please move to the next section in section 5 you will provide information about your household assets if you or anyone in your household has not given away anything of value or owned a home in the last two years check no next to each question if you or anyone in your household has given away anything of value or owned a home in the last two years check yes next to each question and answer the remaining questions your rental assistance specialist will complete the referral source information area by signing this document you are acknowledging that the information provided is complete and accurate please remember to sign and date now let's review the next form the authorization for release of information form gives the louisiana housing corporation permission to share your personal information with another person or agency however you have the right to decide what information is shared how your information is shared and with whom your information is shared with if you would like to share your information with a specific person or agency please list their name phone number and how to share that information in this area of the form if email is your preferred method please check the box to acknowledge that you understand that email is not completely confidential by checking the boxes below i understand you are acknowledging one that you are not required to share your information nor sign this release two that releasing your information could give another person or agency information about your location and three now releasing information would confirm that you have been receiving information in the louisiana services network data consortium database and share with partner agencies the information collected will depend on your situation you may be asked to provide information about your identity housing income legal history medical history and services needed sharing your information will help the louisiana housing corporation identify other programs and services that you may be eligible to receive as well as better coordinate services for you and your household please read this form in its entirety to see what information will be entered into the database and the benefits of sharing that information in the client informed consent release of information authorization portion of this form you have the ability to limit or restrict access to personal information by completing this section if you would prefer not to share your information with a specific program or agency check box one and list the name next to the programs or agencies in the line below if you would prefer to share your information with specific programs and agencies check box 2 and list the name of the programs or agencies on the line below if you prefer to only share your information with the louisiana housing corporation check box three in the my rights area please check all of the boxes by checking box one you are acknowledging that it is within your right to request a copy of the information shared with other agencies by checking box 2 you are acknowledging that it is within your right to refuse to sign this authorization and it will not affect your participation in the louisiana emergency rental assistance program by checking box 3 you are acknowledging that it is within your right to cancel this authorization in writing at any time please include the name and address of the agency that you no longer want to share your information with and by checking box 4 you are acknowledging that the information share can be redisclosed by the recipient it's important to note that this authorization is valid for up to three years from the date listed on this document please sign in date to complete this form your rental assistance specialist will sign after receiving your completed documentation now let's review the next form the self-certification of annual income form is used to document annual income of individuals and families that have lost employment or income either permanently or temporarily due to the covet 19 pandemic please read pages 4.1 and 4.2 for specific details let's proceed to page 4.3 part 1 eligibility the information provided in part 1 of this form will be used to help determine your eligibility to participate in the louisiana emergency rental assistance program now scroll down to the words check all that apply please check whether you are homeless or experiencing financial hardship as a result of covet 19. in the box below describe your financial situation as job loss due to coven 19 reduced income due to covet 19 or share whether you were diagnosed with covent 19. in part two you will provide your contact information including your current address phone number and email address please include your mailing address if it is different from where you live now list everyone who resides in the home including the head of household please indicate their name including first last and middle initial relationship to the head of household date of birth their student status as part-time or full-time and disability status as yes in the area below submit additional information about the students in your household by checking the boxes that apply over the age 24 veteran of the us military married has dependent children or is part of a low-income household now let's proceed to part three annual income for each household member anticipate the annual income for the next 12 months you will do this by converting your current income into annual figures for example if you bring home 300 per week multiply 300 by 52 because there are 52 weeks of the year your anticipated annual income is 15 600 if you bring home 600 bi-weekly which is every other week multiply 600 by 26 because there are only 26 bi-weekly payments per year your anticipated annual income is 15 600 if you bring home six hundred and fifty dollars semi-annually which is two times per month multiply 650 by 24 because there are only 24 semi-monthly payments per year your anticipated annual income is fifteen thousand six hundred if you bring home thirteen hundred dollars each month multiply thirteen hundred by twelve because there are only twelve monthly payments per year your anticipated annual income is fifteen thousand six hundred to determine the total income for the household add up all columns on the last row of this chart please note that full-time students ages 18 years or older excluding the head of household or spouse do not have to report annual income more than 480 dollars leave all areas blank that do not apply for each household member provide information about the annual income received from assets in the assets category area you will need to report the type of asset such as a checking or savings account insurance settlement retirement or personal property also indicate the cash value of the asset and if the asset generates income if the asset does not generate income report zero if you have given away assets within the last two years value at more than one thousand dollars indicate the cash value and the amount of income generated in the disposed asset area please be sure to add up the totals on the last row of this chart if household members do not have assets leave both areas blank as indicated in the box below this section will be completed by the program administrator please continue part 4 applicant certification by completing this section you are certifying that the information provided is complete and accurate the applicant and all adult members of the household should sign print and date this form now let's review the next form you will use the disclosure of identity of interest to indicate your relationship or affiliation with an employee of the louisiana housing corporation or member of congress if this does not apply to you please list none on the name line if this does apply to you please include the person's name and relationship or affiliation to you yes or no in each box if you are unable to answer some of the questions your rental assistance specialist will attempt to obtain this information on your behalf please sign and date this form and request your landlord's signature if you are unable to obtain the landlord's signature your rental assistance specialist will attempt to obtain it on your behalf now let's review the next form the lease addendum will amend your lease to include the provisions included on this document please read the entire document for specific details you will need to complete section c terms of the lease the information required to complete this section was provided during your initial call with the louisiana housing corporation if you are unsure or don't have access to this information please contact your assigned rental assistant specialist for assistance section e security deposit please list xero in this area as the emergency rental assistance program will only cover the rent you will also need to complete section f appliances and utilities please include which appliances are included in your rent or paid by you the tenant section g household members list every member of your household and include their name date of birth and relationship to the head of household please sign and date this form and request your landlord signature if you are unable to obtain the landlord's signature your rental assistance specialist will attempt to obtain it on your behalf now let's review the next form by signing the participant agreement you and your landlord are agreeing to participate in the louisiana emergency rental assistance program your rental assistance specialist will use your lease to complete section a however it is important that you and your landlord read and sign this form if you are unable to obtain the landlord's signature your rental assistance specialist will attempt to obtain it on your behalf now let's review the last form in your packing this form provides
