Years ago I worked at document management company.  There is cool software that can automate aspects of hand-written forms.  We had an airport as a customer - they scanned plenty and (as I said before) this was several years ago...On your airport customs forms, the "boxes" that you 'need' to write on - are basically invisible to the scanner - but are used because then us humans will tend to write neater and clearer which make sit easier to recognize with a computer.  Any characters with less than X% accuracy based on a recognition engine are flagged and shown as an image zoomed into the particular character so a human operator can then say "that is an "A".   This way, you can rapidly go through most forms and output it to say - an SQL database, complete with link to original image of the form you filled in.If you see "black boxes" at three corners of the document - it is likely set up for scanning (they help to identify and orient the page digitally).  If there is a unique barcode on the document somewhere I would theorize there is an even higher likelihood of it being scanned - the document is of enough value to be printed individually which costs more, which means it is likely going to be used on the capture side.   (I've noticed in the past in Bahamas and some other Caribbean islands they use these sorts of capture mechanisms, but they have far fewer people entering than the US does everyday)The real answer is: it depends.  Depending on each country and its policies and procedures.  Generally I would be surprised if they scanned and held onto the paper.   In the US, they proably file those for a set period of time then destroy them, perhaps mining them for some data about travellers. In the end,  I suspect the "paper-to-data capture" likelihood of customs forms ranges somewhere on a spectrum like this:Third world Customs Guy has paper to show he did his job, paper gets thrown out at end of shift. ------>  We keep all the papers! everything is scanned as you pass by customs and unique barcodes identify which flight/gate/area the form was handed out at, so we co-ordinate with cameras in the airport and have captured your image.  We also know exactly how much vodka you brought into the country. :)

Definitely ask for a psychologist referral!  You want someone on your side who can understand your issues and be willing and eager to advocate for you with the beancounters because disability can be rather hard to get some places, like just south of the border in America.Having a psychologist means you have a more qualified specialist filling out your papers (which is a positive for you and for the government), and it means you can be seeing someone who can get to know your issues in greater depth and expertise for further government and non-profit organization provided aid.If seeing a psychologist on a regular basis is still too difficult for you, start with your initial appointment and then perhaps build up a rapport with a good therapist through distanced appointments (like via telephone, if that is easier) until you can be going into a physical office.  It would probably look good on the form if your psychologist can truthfully state that you are currently seeking regular treatment for your disorders because of how serious and debilitating they are.I don't know how disability in Canada works, but I have gone through the process in the US, and specifically for anxiety and depression, like you.  Don't settle for a reluctant or wishywashy doctor or psychologist, especially when it comes to obtaining the resources for basic survival.  I also advise doing some internet searches on how to persuasively file for disability in Canada.  Be prepared to fight for your case through an appeal, if it should come to that, and understand the requirements and processes involved in applying for disability by reading government literature and reviewing success stories on discussion websites.

Trust me, I lived it, you are probably being scammed. There are many doctors who flat out refuse to have anything to do with forms stating that someone is disabled - mentally or physically. I have well documented physical disabilities, and when my doctor retired, it took me almost two years to find a new Primary Care Doctor who would work with me.I had one doctor actually cancel my appointment while I was sitting in the exam room, because he read the documents I had brought and realized I would be needing disability confirmation papers. He did not even see me, just told his staff to tell me to leave.Another doctor saw me, at about the time I had my annual review. He agreed to fill out the form but made it clear that he disapproved of someone as young as me “trying to work the system” … he submitted the form without having done an evaluation of my issues, with the statement that he had advised me that I “could return to work effective immediately” — that amputation of my legs in no way prevented me from being able to carry out the functions of my job as a paralegal or as an accountant.Luckily, the insurance review board was not staffed by idiots. They knew my disability status had never been due to my amputations, and they contacted me directly and sent me a copy of what they received.I was able to get my retired PCP to submit a proper evaluation, and did not lose my disability coverage. I also filed a formal complaint against that doctor with the medical licensing board and with his hospital. He was fired by the hospital and his license to practice in Texas was suspended for 3 years. So he moved to another state.A large part of the problem is because there have been many people who have faked their need for disability, and many doctors let that slide. Often, doctors would accept a patient’s self report of inability to perform, and would sign affidavits that said the person was disabled.It did become so bad that even today, when someone says they are disabled, especially when they have an “invisible” disability, the common response is to not believe them.Why that affected the doctors is that the companies that had to pay out the medical insurance (long term disability on a private insurance claim, for example) recognized that suing the individual patients was not worth the cost. However, suing a doctor for having signed off on a fake disability .. that’s where the money could be recovered.So doctors who will do disability evaluations and certifications actually pay a higher interest on their mal-practice insurance because if someone is faking it and gets past the doctor … the doctor is the one who could be held liableSo yeah - finding a doctor willing to work with you to help confirm or document an actual disability (mental or physical) is not easy. You have to keep looking until you find one.Remember - YOU are the client. You are a critical member of the team. Do not simply accept that a doctor won’t help - find a new doctor. Use that first meeting not only for them to get to know you but for you to talk frankly with them about what you need and expect. Oh, we read them. We base the start of your plan of care on them. As the nurse doing that, I go over everything. The doctor I work for uses it to be sure he covered everything. It's very common to forget something when you have the doctor in front of you. This is my profession and even I do it. We expect you to forget something.Then it gets scanned into your chart, there, forever. I refer back to those forms if, for example, your labs turn up something life threatening and I can't signNow you. Who was that you listed as an emergency contact? Hope it's legible. Heck, I hope it's filled out! ( If it was entered before my time, it might not be. Now, you can't see the doctor without it filled out.)It's so important my practice asks you to re do them every year. Patients hate it, complain about it, loud! But if I had a dollar for every time I couldn't signNow someone in this day and age of fluid phone numbers, why, I'd have several more dogs and we'd all be living somewhere warmer!And…oh, you have another doctor? We didn't know that. And they prescribed what? Did what tests? We don't know if you don't tell us 99.9% of the time. You would be amazed how many patients don't bother to tell their primary care physician such important things like…they went to the ER, had an MRI, see a cardiologist, and..etc and so on. We don't automatically know. We should, but that's another story.Feel that paperwork is beneath you? Are you too busy to fill it out? I see that every day too. You know what that tells us? That you don't value this very much. That you are so much more likely to be non compliant, not take meds, no show for appointments, maybe fib a little….a lot… your lifestyle choices….how, if you take your meds. I mean, come on, you can't even follow directions to fill out paperwork! How do you expect us to take you seriously, when from the very start, you don't offer us the same courtesy.If there are any doctors out there, not reading these things, shame on you. But in 30+ years, I have not seen it. As for the doctor asking you about it, well, mine works very hard to get it right. And even the most earnest patients forget something.

Therefore, the only way a birth parent could reclaim custody of an adopted child is by proving to a court that the decision to sign the relinquishment document was done under fraud or duress.
