Filling out a prior authorization is not a requirement of our practice. Most of us do this to help our patients, and it is sometimes taken for granted.Think about it. It is your insurance company that is requesting us to fill out this form, taking time away from actually treating patients, to help you save money on your medications. We understand that, and usually do our best to take care of them, but unless we have a large practice, with someone actually paid to spend all their time doing these PA’s, we have to carve out more time from our day to fill out paperwork.It’s also possible, although not likely, that the pharmacy does not have the correct fax number, the faxes have been misplaced, the doctor has some emergencies and is running behind, went on vacation, and so on.Paperwork and insurance requests have become more and more burdensome on our practices, not due to anything on the part of our patients, but a major hassle none the less.

As a psychiatrist who runs into this situation with my patients frequently, I know it’s a lousy, frustrating situation.I suggest to those of you who run into a denial from your insurance company— call your insurance company and tell them you were prescribed , and the insurance company denied it. You ask them to send a prior authorization form directly to their treating physician (make sure you have your doctor’s fax number). Ask your physician to fill out the prior authorization form and fax it back.We are inundated with Prior Authorization requests, and they are a hassle to search for and obtain. If you call the insurance company, ask them to fax it to your doctor, they are relatively easy to fill out and fax back. They are also available online for us to fill out, and usually get approved immediately.So, if you can help your doctor with the part of getting the PA, it makes it easier for us to help you get your insurance company to agree to pay for the meds.Lastly, you may want to buy a few weeks’ worth of the meds by paying out of pocket, until they are approved by your insurance.Good luck!

A2AAs sad as it is, I have to agree with Jenny Hall on this one.When you purchase prescription drug insurance (or your company does, on your behalf), the insurance company publishes a “formulary” which details what drugs they cover, and what drugs perform the same function or similar function but at a higher price. Drugs listed on the formulary are known as “preferred” drugs. Drugs not listed on the formulary are known as “non-preferred” drugs.Additionally, insurance companies charge different amounts for generic drugs vs brand-name drugs, since brand-name drugs are more expensive.Finally, most large pharmaceutical companies also have mail order divisions, and will only cover a certain number of retail pharmacy prescription fills. If you have medications for chronic diseases, they will usually only cover a few retail fills before they will no longer cover the medication (known as a “retail refill allotment”). The good news is that you can usually get a 90-day supply through the mail, and you’ll pay less than three 30-day supplies at your local pharmacy.My insurance company has six different pricing models, three for mail order and three for retail fills. Our retail refill allotment is 3. For each, we have a price for generics, preferred brand names, and non-preferred brand names.Not all insurance companies operate like this. Many will simply not pay anything for non-preferred brand name medication. The bottom line is that when you purchase the insurance, you agree to these terms. You agree to let the drug company tell your doctor that he is not being fiscally responsible and trying the generic alternatives to the brand name drugs first, before jumping directly to the brand name drugs.The problem is that insurance companies are staffed by doctors who truly believe that generic drugs work exactly like their brand-name counterparts. Unfortunately, we who are consumers of many different pharmaceuticals (brand name and generic) know that this is not always the case. That never stopped the drug insurance company from following that guidance anyway.As has been noted, you can appeal the decision. Your doctor has to prove that the “approved” drugs that they recommend (generally, generic versions of the brand-name drugs they denied) have already been tried and have been found to be ineffective for your condition. The doctor will have to provide the treatment records that show you tried the generics and they didn’t work.If your doctor is willing to go to bat for you, you can file an appeal and in some cases the insurance company will reverse its decision.If they do not, sometimes the drug manufacturer itself has a discount plan you can use. Additionally, don’t forget to contact your local chapter of the MS Society, which has links and resources for you to use to get assistance with drugs and their cost if your insurance company refuses to pay. Well, I can relate to this question because I had 3.5 months to prepare for my Executive level too, along with my b.com exams. And with my sincere dedication to time table I cleared it all in first attempt.So I would like to share my experience with you, hope it helps.It was end of August and I did not touch my executive study modules. August 25- Result day! Most of my seniors results were decalred and many did not clear. And I saw a rush of students in library with huge reference books dedicatedly studying. And here I was with an attitude, dekh lenge yaar abhi toh exam December mein hai. (Translation: we'll see , exams are only by end of December)But the fear of failure hit me when I saw my seniors who did not clear and a rush inside me said, I can't handle this and I have to clear it in first go. August 25- I sat and made time table adjusting my CS course and b.com too! Here are few tips:First:Make two types of time table: 1. First rough draft for next 3months 2. Concrete time-table for every week or twoSo first make a rough draft of 3 month, for example September for 2 subjects or 3 subjects. Then Oct first 15 days for revision and then next 3 subjects in remaining 15days. For concerte time table:Initially set goal for a week and no matter what, follow it. Trick here lies is that if you set a concert time table for 3months you might miss out a day sometimes due to some uncertainty, disturbing your whole time table. You can't eat all the breads on the plate in one go. You need to complete one bread to eat another and decide can you consume 3rd bread now or little later. So finish your first goal and decide whether you need a revision or you can go for another chapter or subject.For concerts time table if you decide to complete a chapter in 3days keep 1days extra. Extra day is if you could not complete it in 3days you have a day in extra to cover it up without disturbing your timetable for next days. (But don't take that extra day as an excuse to watch movie or for time pass.) And in case if you complete your chapter in allotted three day, bingo! you can continue with another chapter and you have extra days where you can revise the chapters.Second:Cut down on all the distractions for 3months: Facebook, whatsapp,get together,parties,family occasions etc. Sleeping , listening to songs, playing with my pet for10-15 mins, breakfast lunch dinner all this activites was taking a break from studies for me.A movie wont harm but it will take away 2-3hrs. Which you can't afford. So think about it. Third:Make sure you get a sleep for minimum 8hrs. Yes only when you are fresh you can completely concentrate, grasp things well and stay active. I use to sleep for 6 hrs in night (10pm to 4am) and 2hrs in afternoon(3pm to 5pm) Fourth:Make a habit of making notes about whatever you study. If you have a habbit of writing and studing . I don't have this habbit. And I am very lazzy to do this as I found it very time consuming. So what I did was sticking sticker notes in my book itself. Making short bullet point and sticking it near the topic helps you when you revise. Examples given in lectures can be noted this way which can help you recollect the other explaination given in class room easily.Fifth:Keep 2nd weekend and 4th weekend of the month, or any day of a month for revision of whatever you have studied so far. Trust me revision is important as what you have studied in september ,if you don't revise it in October then in by end of November when you open it you can be in trouble. Sixth:Last but not least cheating on your time table is allowed but keep in mind its consequences.I am just sharing my experience. You can alter this as per your requirement. This was my time-table:4am-6am: study for CS6am-7am: break7am-10am : b.com classes10am-11am: break11am-230pm: study for CS & bcom when necessary2:30pm-5pm: break5pm-730pm: classes for CS or library730pm-830pm: break830pm-10pm: study for CS(Break time was for breakfast,dinner,lunch,sleeping, playing with my pet,talking to mom-dad or friends family. But my eyes were always on clock that no time is wasted)PS : everything was not done on exact time as I am not a robot. 5-10mins deviation from time was normal. Initial days will be tough but follow it dedicatedly it will become your routine and then habbit. PPS: I followed it strictly might have cheated once or twice. But made sure the time was covered by shortening my break.And trust me after all this hard work on 25th february when I saw my results as "pass" in both the modules and  b.com results as passed- that's a joy which I can't explain in words.All the best for future.I

Counting out pills is the least time consuming part of filling a prescription. Let me give you a brief walk through (different pharmacies may vary slightly):Prescription comes in the pharmacy (by whatever means).The pharmacist checks the prescription to make sure everything is correct and makes sense. You would be amazed at the number of times I’ve seen prescriptions for rash creams to be taken by mouth, inappropriate dosing schedules, or just sheer gibberish on a prescription form.If the pharmacist thinks there is a problem with the prescription, the pharmacy tech has to call the doctor’s office for clarification. Few doctors are happy to hear that someone is questioning their prescribing ability. There may be nothing wrong the prescription itself, but the new drug may interact with something you are already taking, so we have to get an alternative.Once that is straightened out (which may require quite a bit of phone tag), I, the pharmacy technician enter the prescription into the system.The pharmacist double checks my work.Then, and only then, do we run your insurance. It may go through the first time, it may not. It may require a prior authorization. The drug may be covered, but only for a certain number of doses per month. We pharmacy techs have to work all that out and navigate the maze of your insurance - assuming we have up-to-date information. Please let us know if you have changed insurance.Once your insurance has been processed, your prescription is added to the queue. Yours is not the only prescription. They are generally filled in the order they came in, with some priority given to people who are waiting.Your prescription has worked its way to the front of the queue. A pharmacy tech goes to the shelf, pulls the drug off the shelf, checking to make sure it is the correct drug in the correct strength. Bar codes are scanned, a label is generated, pills are counted out. Counting out pills may look easy, but some of those suckers are bound and determined to cause a problem. Let me pour a bunch of BB’s on a tray and see how fast you can count them out. Yes, there are pills that are shaped like BB’s. Then there are the fragile pills, the capsules that want to stick to the tray due to static, the tiny pills, the capsules that want to come open. That’s just pills. There are liquids, creams, drops, and pastes that have to be mixed at the time of dispensing.The label is put on the bottle. The pills are put in the bottle. The bottle and the associated paperwork are sent to the pharmacist for verification.Once the pharmacist okays it, the vial(s) and associated paperwork are put in a bag to await you. If for some reason, the pharmacist finds a problem with it, we have to go back a couple of steps and try again.Keep in mind all this is going on while other patients are coming to the counter to pick up or drop off prescriptions, the phone is ringing, people are coming through the drive-thru, people are asking us where the toothpaste or hair dye are, people wanting to know how long they will have to wait for a flu shot. I’m exhausted just thinking about it.All that so that you can get the correct drug, in the correct form, in the correct dose, in the correct quantity and grumble at the pharmacy staff over having to wait a whole 20 minutes for your prescription. This is not fast food. If we screw this up, we could kill you. Cut us some slack. Drop off your prescription, go run a couple of errands, and pick it up later - and don’t yell at me because your copay went up. Go yell at your insurance company. They are the ones who make that decision, not the pharmacy.
