This is partially an outgrowth of more universal “therapisting” skills—being able to be very present with a client, then step out of that headspace after a session ends; finding ways to track the important details of another person’s story; knowing what it takes, for you to do at least a short-term “reset,” after a difficult or intense interaction, etc. It’s hard to give a good general answer to this question, because different therapists will use different approaches, but I can give you some of my own particular tools and strategies.One answer is: some of us don't. Many of the people I encounter who teach therapists, conduct research, make a decent living as trainers or authors, etc., will speak about the ways in which they really enjoy clinical practice, but perhaps have a cut-off point of a dozen clients, before they start to feel like they're worn out, and not offering optimal client care. So they add in some of those other things, to allow them to make a living, without overloading on therapy sessions.I really enjoy direct client care, and I usually feel energized by it. That's a great thing, in terms of allowing me to take on a larger caseload. It also means I have to be very intentional about setting intentional, reasoned limits for myself, and not relying on my emotions to tell me whether I have room for more people on my caseload (my emotions will always say, “SURE, let's just schedule people back-to-back until midnight!!”). Part of not being overwhelmed is—not building a caseload that is overwhelming.Something else that's been important for me is having carefully chosen and well-established procedures, for responding to client emergencies. I have a very solid, research grounded protocol for treating both chronic and emergent suicidality; I have a fixed and specific process for assessing when to call emergency services for a welfare check; and I have woven those considerations into my informed consent, so that clients know to expect that they will be a part of their care, when necessary. I have also developed a pretty comprehensive set of standards for distinguishing a high-risk crisis situation, from an experience that may be deeply unpleasant, but is not dangerous. That's important to therapisting, in general, but can be especially salient when husbanding my energy throughout a full day of client care. If I feel like I have to make follow-up check-in calls to twenty people, I'm not going to have much time or emotional energy left for anything else—and the clients who actually just needed a text message reminding them of the coping strategy we planned to try this week, or a confirmation of their session time, are not going to benefit from my overfunctioning.In terms of documentation, I use a system that works for me, and might be useless to anyone else. I use a paper appointment book for scheduling, and tracking attendance, payment, noting crisis intervention and mandated reporting, and tracking client contact and clinical consultation—because I have some awesome ADHD, and writing things by hand sticks them in my brain better than typing. I also keep a notebook that I use to schedule and annotate my entire life. It gets reformatted regularly, as things change, and I need new approaches to accommodate them. This is the notebook where I'll do things like list reminders of all the client contacts I need to follow up, along with relative urgency, and information I might need, to do so; lay out my client contact schedule for each day; plan various office tasks, and so on (It's also where I do things like make grocery lists, keep a journal, doodle in meetings, plan professional presentations, lay out my vegetable garden, etc., etc.). I keep a second notebook for psychotherapy notes, which I use in-session. And I have a hilariously elaborate system of formatting and annotation, that helps me visually scan those notes quickly, to pull out tasks or communication items for follow-up, notes on any crisis assessment or intervention, specific topics for follow-up at the next session, and so on.I'm also very intentional about transitional moments, between clients. I don't always have an unstructured break after a session—some people swear by this, but as long as I can catch my breath every two or three hours, I do just fine. I do allow time, and take my time, in saying goodbye to the previous client, greeting the next client, offering a cup of tea or bottle of water, getting a drink myself. I find it important in client care, to build in a clinical “warm-up” and “cool-down”—who, exactly, wants to just plop down on the sofa and talk about their trauma for 59.5 uninterrupted minutes, you know? And for me, those are the orienting moments that help to close my focus on one person, and engage my focus on the next.Beyond that—while I'll often review a previous session’s notes or progress report before my next meeting with a client, it's actually kind of amazing how well situational memory works, to pull up detailed bits of information. I might not be able to tell a client’s life story off-the-cuff, if you caught me on my day off—in fact, reschedules to the “wrong” time of week can really throw me. But when someone sits down with me, in their usual seat, at their usual time, we really can usually just pick up where we left off.It also helps, of course, that therapy isn't just a conversation; it isn't even just a clinical relationship. Therapy is a treatment process, with specific outcomes in mind, and usually with structured approaches to getting there. Some therapists are very much wedded to a particular theory or clinical approach; some, like me, are a little more fluid—but most of us have a fairly predictable way of working, and sense of how client progress is unfolding. It's much easier for me to remember and engage deeply with the complexities of someone's story, as informed by the work we’re doing, than to do so with a friend, over coffee, when we’re just hanging out for funsies. That's not because I don't care about my friend, because I do—but what we’re doing is less structured, less goal-directed, and much less likely to fall within a fixed, familiar trajectory of interaction and progress.
