Not at the diagnosis itself but getting here has been kind of amusing.A few years back, a doctor looking at a chest x-ray noticed that I had a healed crushed vertebra. I knew that I had hurt my back a year earlier but didn’t realize it was a crushed vertebra. When I told her that I got it lifting a shop-vac into my pickup truck, she was suspicious. As a (then) early 50’s male, I shouldn’t have bones getting crushed for something that innocuous. She ran some tests that showed that I had abnormally low bone density but everything else was fine. She suggested that I see a specialist.After a (long story) while I get to see an hematologist/oncologist. They start doing some measurements of protein in my blood. The value keeps rising. Bone marrow and biopsy tests from my hip shows unusual cells. The doctor says I have Waldenstrom’s macroglobulinemia. But … they keep testing and the protein value roughly stabilizes. It’s too high but not high enough for a definitive Waldenstrom’s diagnosis. I have Monoclonal Globulinemia of Unspecified Significance, MGUS. Treatment is to watch and wait to see if it develops into something more serious.This summer, I crush another vertebra lifting up a computer. MRI scan shows a tumor in the crushed vertebra and the vertebra next to it as well as some other damage. Diagnosis? Multiple myeloma.I get signed up for a clinical study for multiple myeloma. Doctors need to get another bone marrow and biopsy from my hip to baseline my condition before starting treatment. Diagnosis? Not multiple myeloma. It’s Waldenstrom’s.The hematologist wants to have a back specialist take a look. When I see the very experienced back specialist he says that he’s never seen Waldenstrom’s lead to crushed vertebra. Diagnosis? It’s probably multiple myeloma.But, importantly, he says we can’t keep guessing. We need to get a bone marrow and biopsy test from the vertebra to be certain. Result? It’s Waldenstrom’s. Everybody is now in agreement.I know way more about my immune system than I ever expected to know.

To obtain the information needed to complete a treatment plan, a health worker must interview the client. The information gathered during the interview is used to write the a complete treatment plan.Steps to proceed when setting up plan:A: Mental Health AssessmentGather information. A psychological evaluation is a fact-gathering session in which a mental health worker (counsellor, therapist, social worker, psychologist or psychiatrist) interviews a client about current psychological problems, past mental health issues, family history and current and past social problems with work, school and relationships.Follow the sections of the evaluation. Most health facilities provide the mental health worker with an template or form to complete during the interview.Note behavioural observations. The counsellor will conduct a mini-mental-status exam which involves observing the client's physical appearance and his or her interactions.Make a diagnosis. The diagnosis is the main focus. Sometimes a client will have multiple diagnoses such as both Major Depressive Disorder and Alcohol Use. Diagnoses must be complete before a treatment plan can be completed.B: Define goalsDetermine signNowable goalsOnce you have completed the initial assessment and made a diagnosis, you will want to think about what interventions and goals you might want to create for treatment.Think of interventions. The interventions are the meat of change in therapy. Your therapeutic interventions are what will ultimately evoke change in your client.Discuss goals with the client. After the initial assessment is conducted, the therapist and client will collaborate to create appropriate goals for treatment.Make concrete goals for treatment. The goals are also what make up a large component of the treatment plan. Use the SMART methodC: Setup the treatment planRecord the treatment plan's components. The treatment plan will consist of the goals that the counsellor and therapist has decided on. Many facilities have a treatment plan template or form that the counsellor will fill out. Part of the form may require that the counsellor check boxes that describe the client's symptoms.Write down the goals. Your goals need to be clear defined. Remember the SMART goals plan and make each goal specific, measurable, achievable, realistic, and time-limited.Express specific interventions you will use. The counsellor will include treatment strategies the client has agreed to.Sign the treatment plan. Both the client and the counsellor sign the treatment plan to show that there is an agreement on what to focus on in treatment.Review and improve as needed. No plan in perfect. You will be expected to complete goals and make new ones as the client progresses in treatment. The treatment plan should include dates in the future that the client and counsellor will review the progress the client is making. What is your most memorable experience of working your first job?My first job was working at a coffee house off campus near my university. I was a barista, cashier, and when needed, I pitched in on cleaning and running the dishwasher. I often closed the cafe at 2 am and met some interesting characters late at night. It was a fun job.One evening, a “friend” dropped by the cafe and gave me a brownie. I’d been working on the cash register which wasn’t rocket science, but it still required paying attention in order to make change. I swear on all that’s unholy or not that every bit of pot my friend had baked into the brownies ended up in the square he gave to me. Not good. Not good at all.I finally had to go to the shift manager and let him know I’d need to switch positions for the night. Luckily he was cool - he took one look in my eyes and started laughing. He put me to work on the dishwasher.I truly felt like I was part of the machine, like I was one with the dishwasher. We were intertwined, the machine part of my flesh, the water jets and the steel part of my body. When I finally started coming down from the high, I realized my co-workers were looking at me like I was a two-headed zombie. They said I’d been in such a zone that I’d actually run a cycle in the dishwasher with no dishes - I’d moved so fast, there was nothing left to wash, so I just started it up again with nothing inside. I’d been like that for over two hours and anytime someone tried to talk to me, I’d only reply with “Can’t talk, coming down.” (That’s a reference to an old Simpson’s episode after Lisa was dosed with LSD at a country fair.)I was supposed to close that night, but my shift manager sent me home early for some reason. I apologized profusely after telling him about the brownie. I promised I hadn’t known about the pot until after I’d taken a couple bites, and that I’d never thought it would have effected me so hard.
