It's very common to forget something when you have the doctor in front of you. This is my profession and even I do it. Do people fill out online pre-registration forms (in-take, medical history, etc.) before a doctor's visit? Why or why not?
Yes, I do every time it's available, for a few reasons:I type much faster than I write.I experience less arthritis pain when typing vs writing.I would rather fill out paperwork at home vs struggling to balance a clipboard at the office.I hope that less paper will be wasted by using online forms.My doctor will usually be able to understand what I've typed but not always what I've written.Online forms look neater and I am soothed by neater.I hope to spend less time in a waiting room.I imagine that my typed form less likely to be misplaced or lost.I'm sure there are plenty of other reasons. I see no reason to avoid an online form, unless you had some strange paranoia about using a computer.
