the pre-review questionnaire commonly referred to as the prq is a survey tool or questionnaire completed and submitted by a participating hospital prior to a site visit the prq is web-based and issued to a hospital following submission of the application via the acs quality portal or q port for qvp there are two types of prqs first is the hospital prq this is generally completed by the surgical quality officer or designee this prq asks questions and requests documentation in regards to the surgery-wide quality infrastructure at the hospital the second type of prq is the specialty prq specialty prqs are short surveys designed to address the specialty level standards such as disease-based management the specialty prqs are completed by the specialty or division chair for each participating specialty for a focused visit this would be two specialties or for a comprehensive visit there would be one for each surgical specialty offered at the hospital the prqs follow the order of the qvp stand
