I used to write software to help hospitals keep track of their efficiency, and wait times was one of the things measured. (So were unnecessary tests, length of stay in Hospital, re-admission rates and much more)If you are interested in wait times and getting reports, the Canadian Institute for Health Information CIHI is a valuable resource.There are some things to consider.Triage: Some thing are important (Breathing before Bleeding before Broken) than others, so wait times should be sorted by diagnosis before making claims.Past results show that when Canada sets a target, the system responds to meet it, but you can't optimize everything. Triage as a concept is about setting priorities.Also, consider that workload on the health system is not uniformly spread out. I once broke a bone on Dec 23. I was out of the hospital with everything done, after having an X-ray, (including a copy of my x-rays on CD because they knew I was going to travel after Christmas), my physician talking to a specialist about the results deciding not to do surgery. All done in 3 hours from the time I got *injured*. Yes, that includes getting up, going home, finding a babysitter for my kid, waiting for my wife to come home and bring me to the hospital, limping across the parking lot, going home afterwards, etc. Everything. Apparently Canadians aren't getting sick or injured the Thursday before Christmas Eve. There are other days of the week or year where the emergency rooms are overwhelmed, and wait times skyrocket for a short period. Disease or cultural patterns can make some days have a sudden surge in need for health service.One important indicator that we track is the time from arrival to triage. If we can quickly sort out whom is going to die and whom can apply ice while waiting for the critical cases to go first, we get a preferable trade of higher wait times for non-emergency care and low overall mortality.Some Doctor's practices are better at managing appointments than others. So beware making conclusions about overall performance from a few samples, a classic logical fallacy. With my family Doctor, if you are sick or injured, you see him that day. If you just want an allergy shot or to follow up on a chronic condition, then it is next day or day after scheduled appointment. My wife's doctor (different practice) is nowhere near such schedule control and punctuality. [Any statements about why given the data I have would be purely speculative.]Population Density and Size. It is easier to achieve economies of scale and the efficiencies of specialization with high population and density. It is also easier to reallocate patients to another hospital with available capacity if it is nearby. Those whom compare Canada to France or the UK have to remember those countries have double Canada's population stuffed into less space than Manitoba, just 1 province. If those countries can't run a more efficient health care system, there is something very wrong with them.Pay by Taxes? Well that is a 2 sided coin.Canadian political parties both left and right claim to have plans to have balanced budgets. (This should be taken to mean that the Canadian voters expect the parties to at least claim they have a plan to eventually do so, and is not a partisan endorsement or prescription of how that is to be achieved.)This may come as a surprise to some readers, but a sizable number of Canadians want to keep their taxes low, and sometimes vote in governments with Tax Cuts as their agenda. This often does result in cuts to health care service, such as the Ontario Liberals removing annual eye exams from coverage (around 2004) . So while we do have taxpayer funded health care, it should not be read as unlimited money. We are trying to get value for money. Moving to a public system and away from a private one is sustained by being better value for money. Even those wanting to expand the public health care system are expected to bargain hard to control costs.Here is a hypothetical to make a point: Suppose you have to buy expensive diagnostic equipment, do to 48 "tests" a day. One unit can do one test an hour. As the purchasing manager, do you buy 2 of them, and run 24 hours a day, or 4 and let the techs sleep at night? If you buy 4, you have spent twice the money to achieve the same speed of service. If you can't line up the people needing the test to fill that queue 24 hours a day, you can't get away with buying only 2. Idle capacity costs money. If there is always 1 new person in line to get service as a test begins, you can maximize throughput and eliminate down-time. To a certain extent, wait times are about minimizing idle capacity.Next time you are in a hospital ward and there is no nurse at the station, remember that admins want their nurses doing patient care over 80% of the time. So 80% of the time they are out doing rounds and actively taking care of people instead of being available at the station. One will eventually return to the station, and one will definitely respond to the emergency button.Cuts that remove idle capacity are often called 'trimming the fat' and don't impact wait times. Other cuts do eliminate capacity or fail to provide service, so there are other issues. In some ways, this is dumping costs off the taxpayer and onto the sick, privatization by stealth.This is to illustrate that there is a tension between financial efficiency and speed of service. Sometimes we make good choices to achieve both. There are also cases where improving patient throughput at a lower cost per patient raises total costs because more people can get service faster. In the spirit of a non-partisan forum, I'll leave it to the Canadian readers to decide which politicians at a particular moment are cutting services to achieve budget targets and which ones are finding efficiencies and yielding real savings.Then there is something to consider: How much surplus capacity is needed in case of emergency, seasonal variance, or a crisis that can quarantine hospitals, such as SARS?In part America is paying for surplus capacity, UN-required tests, profit margins, massive administration, advertising, billing collection and bad debts, and other business overheads. Unlike America, other countries try to do bulk purchasing to negotiate better deals for drugs. This is part of how other countries can spend a lot less per citizen on health and get comparable or better results.In general Canada does quite well at giving priority to doing things to save lives faster rather than elective procedures. Remember when comparing health care systems you have to use %of GDP as a measure, since a government dumping a cost off the Taxpayer and onto the taxpayers whom happen to need care helps the Government's bottom line and reduces taxes, but does not make the national system more financially efficient.

Sounds like a question an American would ask.In Canada our universal medical insurance system is the third rail of Canadian politics.Any politician who suggested even weakening it a little let alone eliminating it would be railroaded out of office in an eye blink by their own party. They would be out faster then Jeff Sessions was fired after the 2018 US election.In a parliamentary system like we have, the leaders of each party serve at the pleasure of their party and can be replaced without a new election. The prime minister is merely the leader of the party with the most seats in the House of Commons. They are replaced when necessary.If a party decided to try this stunt, this would trigger a free vote of non-confidence in Parliament and the government would lose which requires a new election. generally within 60 days. Canada has flexible elections dates in case of such things and short campaigns.In the election, it is unlikely that a party proposing such a thing would win a single seat. Like 1993 or so when the Progressive Conservatives dropped from about 156 out of 295 seats to 2 seats in the election.The Progressive Conservatives died with this election and it took over 10 years for a new viable Conservative party was established.My point is that such a thing would be political suicide for whoever mentioned it.Unlike Americans we know our medical system works as well as the US system except for the top 5% of Americans. And way better than the bottom 30% of Americans get.It is not perfect but we keep trying to make it better all the time while not sacrificing care.

Did you know that a million dollars in U.S. currency weighs just ten kilograms? It's true. A freshly-minted $100 bill weighs in at slightly over a gram, and 100 of them is ten thousand dollars. 100 of those stacks, and there's your million.It’s not often that 10 kilograms - 22 lbs of anything can change your life. But on February 25th, 2014, that’s exactly what happened.  Day 1: $1,000,000 As the man in the gray suit walks away, I shout after him “Hey, come back here. Who are you? What’s this all about?” He does not look back and quickens his pace. Between the choice of chasing down a stranger, or securing what appeared to be stacks of currency, I chose the currency. We can resolve the issue of his identity later, but a loose sack of cash is, well, a loose sack of cash. I look through the contents again. Bundles of US$100 bills, stacked a hundred bills deep, wrapped in standard $10,000 bank bands. A quick count revealed that there were precisely a hundred of those stacks in the bag, and spot-check riffle-counts of the $10k bands suggest that there are no short-stacks within. These were full bands of $10,000 apiece of non sequential USD$100 bills, and I was holding what appears to be a million even in cash. And it feels like just as many question are swirling in my head, as I feel my pulse pounding in my skull. Who was that guy? Why me? What is this all about? But the most urgent thoughts swim past the dizzying deluge of unanswerable questions. Fakes. It’s one thing to inadvertently be the recipient of counterfeit currency; as you’re reading this very sentence, a clerk at a retail store somewhere in your city just accepted a counterfeit bill and made change from the real money in the till.  But to be in possession of a life-changing amount of counterfeit currency of the United States of America? If there is a GPS tracking device embedded in its seams, it would take too long for me to root it out. Better to incinerate it, and make sure that whatever trail it was laying stops at a dead-end for its pursuers. I pour the stacks of bills into an empty duffle back from my garage, and lock the bag in my condo. There’s an abandoned marina just a mile from my home and I get in my car and drive straight to the docks, at the top of the posted speed limit. After pouring enough Kerosene on the bag to see the shimmering mist of petroleum evaporate above it, I lit a book of matches and threw it in the middle of the mass. A satisfying “Whoomph” lights up the fire, and I watch the edges of the bag curl and burn - sizzling in the midmorning sun. If it was fake, they’d find a way to stick “possession of counterfeit currency” charge on me, and I'll be shoved into a Federal concrete box, draining the best years of my life away, only to be released when I can’t chew solid food any more. By necessity, he has a high-end currency counter/ counterfeit detection device in his office, which can swiftly count and verify large sums of money with precision. “Bobby, it’s me.” “Hey, what’s up.” “Can I come to your office - like right now?” “Sure, what do you need?” “I, uh, came into some money. Long story, and I really don’t want to get too much into the details … but I’m wondering if you’d be willing to run the bills through your counter for me? I’m not 100% sure they’re real, and I’d like a discreet way of verifying them. If they are, I’m going to pick up some bullion as well.” “Sure man. Happy to help. How much money are we talking about?”“$60,000” I flinch at that - I hate lying to friends, but at this point, I have no idea who to trust. Though if you want to be technical about it, I did come across $60,000. I am just simply not telling Kendrick about the other $940,000 that accompanied the $60k in the satchel that dropped into my life just three hours ago. “Come on by.” I pull apart a few $10,000 currency bands and start plucking random $100 bills from the middle of every 10k stack to assemble a randomized sample of the entire million. 100 bills, wrap it up. 100 bills, wrap it up. 100 bills, wrap it up. Three bands, thirty thousand dollars, randomized and fully assembled to be tested for authenticity. “Half” of my alleged $60k windfall. The rest of the loose bills are refolded back so there remains 97 stacks of $10k racks, re-wrapped and properly sorted. In 30 minutes, I will figure out if I’m rich, or holding on to enough illicit contraband to send me to Federal Prison for the rest of my life. =============================The Golden Path, like most bullion dealers, work out of small, highly secured office covered by multiple layers of security. At any given moment, Robert may have several hundred thousand dollars in cash or gold, silver and platinum bullion on the premise, it pays to be careful. One of the few civilians in California with a Concealed Carry Weapons permit, Kendrick and I met on pistol gun range ten years ago; we bonded over shooting .45 ACP slugs down-range. While his clientele is mostly legitimate, I’m certain the most lucrative of his customers are criminals - and he smart enough to know not to ask the sort of questions that open up a line of liability for him.  So long as the proper theatrics of anti-money-laundering protocols are observed, everyone is technically in the clear. I hand him the three $10k stacks and he pulls the bands off them and puts the entire block in his high-speed currency counter. After a second, the machine spools up and the digital counter swiftly runs from zero to three hundred. Thirty thousand dollars. “It’s real.” It’s real. His words hang in the air for a moment, and it takes a moment for them to sink in. One million dollars. I’ll be right back.” He counts back $20,000 and hands it to me, taking the $10,000 in the back room of his office and returning with the 5oz Swiss bar and two heavy 1oz American Eagles, along with a receipt for US$9675. I pause for a moment and hand him back one of the $10,000 stacks. “I know the limit is $10k in transactions per day. Consider this pre-payment for a purchase tomorrow. Your call, on a mix of anything up that totals up to $9500. You will need a copy of the incorporation papers from your company and a copy of the EIN letter from the IRS.  You will also need your Canadian passport or some other form of Canadian identification to show that you are not a US citizen, unless you happen to have a US social security number or ITIN.   I will warn you that US banks are subject to such strict reporting laws now, it can be difficult finding a US bank that knows how to set up a new bank account with a non-US signer on the account, so it may be worthwhile to make some phone calls to be sure you talk to someone that can actually help you.  You might even want to start with the Canadian banks that have US affiliates as they can sometimes kick-start the process for you and be sure you are in contact with someone who knows how to properly fill out the paperwork.

I plan to migrate to Canada, and I’m Singaporean. Here are some reasons, and if you can relate, you probably will find it a good idea too:Boredom - Singapore is small and gets boring very quickly. Canada is the second largest country on earth with huge diversity of different cultures, languages, ethnicities and nature. An hour away from Calgary and you’re in banff. In Singapore you can drive from one side to the other in 45 minutes, while it’ll take maybe even a day to drive across one province in Canada. There was a guy who tried to find one person from every country in the world in Toronto, and he did it.Stress - life in Singapore is stressful. I suspect it is the same for the biggest cities of Canada, where I’ll most likely be immigrating to. However Canadian cities consistently rank high on livability indexes not to mention the amazing social benefits which are close to non-existent in Singapore. For example, universal single-payer healthcare, more paid vacation, social safety net/welfare benefits etc. Singapore is small and needs the very best talent of the world to survive as a country, thus the competitiveness. In Canada, I heard companies require less qualifications and more ‘Canadian experience’ for immigrants - the opposite of in Singapore. In Singapore, if you have a good job, good. If you’re unemployed, good luck to you.French - I speak french. Toronto and Vancouver already have huge numbers of immigrants. Quebec does but not so much. And french language skills is an asset both for passing the immigration criteria to Canada as well as for jobs in Ontario and New Brunswick. I love the québécois accent and food.Progressive attitude - this is a touchy subject with relation to politics but I find that Singaporeans in general are much more close-minded than Canadians are with regard to things like taxes, welfare, LGBT, sex, and less materialistic. Singapore might be a developed nation, but the mindsets of most people remain very conservative and Asian-oriented traditional beliefs very much prevail. I am a hardcore liberal (maybe center-left by Northern European standards) and Canadian politics lean much more towards mine than singapore’s. It’s tough always having no voice to express one’s opinion when all that will do is getting labeled as a rebellious, ungrateful and spoiled teenager.Proximity to the United States - I’m very interested in the United States too and I’d like to explore both countries as much as possible. Road tripping across North America just isn’t possible if you have a full time job in Singapore.(Edit) Identity - in Singapore everything is labeled by race, ethnicity and heritage. Filling out forms for government services always has a ‘race’ checkbox. I dislike such segregation and distinction. Chinese privilege exists, despite how the government likes to promote racial tolerance and integration. Canadians are far less racist and critical of one’s ancestry in my experience. I’m also expected to behave like a proper chinese by mainlanders and elders, by speaking mandarin, thinking conservatively (by this I mean selfishly and only in the interest of the family), following meaningless traditions and weird superstitions. In Canada, I can just identify as Canadian. Nothing more to it. I aspire to be a human being whose nationality is Canadian.These are just some of the reasons. Canada is a very compelling county for me because I feel like I enjoy much more personal freedom with relatively minor trade offs. Canada is only marginally less safe than Singapore, just as stable and secure, and in relation to cost of living and salary a cheaper place to live. I’ll add more when I think of them.
