What happens during an intake speech evaluation session for a young child?
Depends on the setting and the student. In US public schools, after all the initial paperwork is done, the SLP typically plays with the child and tries to elicit some speech from him/her. It's important to know what the child's spontaneous utterances sound like and how those utterances parse. Unless the evaluation includes a hearing evaluation performed by an audiologist, the SLP will do a hearing screening on the child to see if such an audiological evaluation is warranted. If the child's developmental level permits, the SLP will administer some standardized tests which involve a lot of pointing at pictures and saying things in response to pictures. With very young children, there are even a few standardized tests that involve object manipulation and parent report. Standardized tests can be used to sample a lot of speech sounds and a number of specific language skills in a short time, and are also useful in comparing the child's performance to the performance of the (hopefully large number of) children in the norming sample that is used to create the test. Finally, standardized tests provide scores that can qualify a child for school-based speech therapy under state and federal laws. The SLP will also examine the child's mouth. The SLP will have the child say "ahh" so he can check for the movement of the faucal pillars (those arch-like structures that flank the space between the teeth and the back of the throat) and soft palate (aka the velum, from which our wriggly little uvulas hang), which helps us make "k" and "g" and also separates the oral and nasal cavities so our speech doesn't sound too nasal. If the child just doesn't DO standardized tests, the SLP may play with the child with a tape recorder nearby, or better yet a video camera with a microphone, to record a speech and language sample. Such a sample can useful for evaluating everything from pronunciation to sentence structure to stuttering to voice production. The parent is almost always present during the evaluation, especially if the child is very young. If the parent is a distraction, he/she can watch from behind a 2-way mirror if the facility is well-funded enough, or sit behind and away from the child if the setting doesn't have two-way mirrors. But I generally have parents in the room with me during the evaluation, since their presence makes the child more comfortable. After the evaluation, the SLP will probably talk to the parent about the observations, but evaluating test results and language samples takes time, and will have to wait for a subsequent meeting. There are legends floating around that this kind of testing is highly stressful for most children, but in the real world, this only happens if the child is extremely shy, which is not common in my experience. For most kids, the testing is just another set of activities that the grownups have them do. Children like getting little rewards such as stickers for doing this kind of work. BUT if you're dealing with a very young child, say three years old or younger, you may be dealing with a child who gets antsy, sleepy, or clingy after about a half hour of testing. A wise SLP will spread the testing out over a number of days for such a child, with no more than 30 minutes of testing on a given day. Obviously, the procedures I have outlined don't work on EVERY child. For example, children with motor disorders that are signNow enough to prevent speech and impair signalling should be tested by SLP's who specialize in augmentative and alternative communication. Another example: If pronunciation and spoken language tests that are designed for and normed on blind children are not available, the SLP may need to depend heavily on his/her analysis of the speech/language sample. For children with complex needs, the SLP may simply be one of a number of specialists who test the child. In hospital settings, the team may include an M.D. and mostly Ph.D's.
