Somewhere on that form should be a blank for routing number and account number. Those are available from your check and/or your bank statements. If you can't find them, call the bank and ask or go by their office for help with the form.  As long as those numbers are entered correctly, any error you make in spelling, location or naming should not influence the eventual deposit into your proper account.

In the US, as long as you're not applying for a job which requires you to drive, it should probably be ok.  Most employers use a driver's license or State-issued ID card as a way of establishing that you're actually who you say you are. If the job does require a valid driver's license, you aren't qualified for that position yet.  The job posting may have listed minimum requirements.  But if you're working at a desk or front counter or in a restaurant (not delivering), the lack of a driver's license probably isn't a big deal.

Some thoughts:Temp agencies - There are several “lower-rung” temp agencies which will hire people who are essentially homeless. I hate to give the primary one’s name; but let’s just that they are “Ready for Labor”. You can quickly earn a few dollars working for them and you can also pick up side jobs from people who don’t want to pay the extra to the temp service.Day labor - A lot rougher than temp work and you risk getting ripped off. However, it’s cash in your pocket and you can do as much or as little as you want or need to do. This is mostly construction, landscape work and light and heavy cleanup. Again, the primary caveat is that some employers will cheat you and since you are working off the books there is little that you can do to prevent this from happening.Veterans services - Every major city has an Veterans Administration representative in their unemployment office. This person will help you get assistance, look for work and they often have contacts which will provide you with a low cost, or free, place to stay. You do need to be a veteran of military service and you do need to have an honorable, general or a medical discharge to avail yourself of their assistance.Local churches - There are numerous local churches which will help you if you demonstrate that you are willing to work to get back on your feet. If you aren’t religious, you may need to keep that fact to yourself; and most will require that you attend their services while they are helping you. However, they often will be very patient with you and many can assist you in finding work fairly quickly, if there is work available in that area.The Salvation Army - The premier agency when it comes to assisting the homeless. They have all of the local contacts and they will almost always do an outstanding job helping you get back on your feet. They do require that you desist from drinking and drug use while they are trying to help (although they can help you get into a free rehab program if one is available) and they may require you to attend a religious service or two. However, they also will work very hard at helping someone who is temporarily down and out to get back on their feet again.There are a few other options, but the above are the best places to begin a job search if/when you are homeless, in my experience.
