I received this message after registering the slot..Also I have downloaded the form but it says this..Can someone tell me if a normal physician can sign this form ?

There are a different number of characters on the license for every state. Some states have 6 characters, some have 9 or 10. Trust me, I lived it, you are probably being scammed.
