Not at the diagnosis itself but getting here has been kind of amusing.A few years back, a doctor looking at a chest x-ray noticed that I had a healed crushed vertebra. I knew that I had hurt my back a year earlier but didn’t realize it was a crushed vertebra. When I told her that I got it lifting a shop-vac into my pickup truck, she was suspicious. As a (then) early 50’s male, I shouldn’t have bones getting crushed for something that innocuous. She ran some tests that showed that I had abnormally low bone density but everything else was fine. She suggested that I see a specialist.After a (long story) while I get to see an hematologist/oncologist. They start doing some measurements of protein in my blood. The value keeps rising. Bone marrow and biopsy tests from my hip shows unusual cells. The doctor says I have Waldenstrom’s macroglobulinemia. But … they keep testing and the protein value roughly stabilizes. It’s too high but not high enough for a definitive Waldenstrom’s diagnosis. I have Monoclonal Globulinemia of Unspecified Significance, MGUS. Treatment is to watch and wait to see if it develops into something more serious.This summer, I crush another vertebra lifting up a computer. MRI scan shows a tumor in the crushed vertebra and the vertebra next to it as well as some other damage. Diagnosis? Multiple myeloma.I get signed up for a clinical study for multiple myeloma. Doctors need to get another bone marrow and biopsy from my hip to baseline my condition before starting treatment. Diagnosis? Not multiple myeloma. It’s Waldenstrom’s.The hematologist wants to have a back specialist take a look. When I see the very experienced back specialist he says that he’s never seen Waldenstrom’s lead to crushed vertebra. Diagnosis? It’s probably multiple myeloma.But, importantly, he says we can’t keep guessing. We need to get a bone marrow and biopsy test from the vertebra to be certain. Result? It’s Waldenstrom’s. Having worked in the Health Insurance field, both on the end of the Health Insurance Company and on the billing end. I have to disagree somewhat. It completely depends on what insurance carrier you have, what time of year it is (at the first of the year, when policies change, things tend to be slower b/c not all of the plans have been built and fully tested and given the green light yet). Also, it depends on what information is in your claim. Was it a PCP visit for the flu? Was it for a mastectomy due to (confirmed) breast cancer? Was it for a heart transplant? Were you having nonmalignant lesions removed? These all play a part in the processing as well. Even if a certification/authorization was obtained prior to surgeries, it is not uncommon for the Ins to want a copy of the operative report/notes, etc. And more times than not, these are sent out to be dictated, so then you have to calculate the amount of time it takes for that info to get to and from where it has to go, before it can even be sent to the insurance carrier. Also, was the claim a "clean" claim? Meaning there were no errors on the claim. Accurate DOB, Member ID, valid DX/CPT/HCPC codes, was it sent to the correct payer/insurance company? Were the Diagnosis codes allowable for the CPT/HCPC codes billed? If not, the chances are either A: the claim will deny or B: Ins Co will send out a letter advising you of the problem. Also, did you provide your doctor with a new insurance card? Or is the card they have on file old and therefore may have old Member ID's, Group #'s, Payor ID's? Another factor is I see frequently is have you been sent AND have you completed and returned an OIC form? If you have gotten one, but didn't complete it, then your claims will stop paying until you complete that and return it (this is something that happens on a daily basis sadly). Ins. Co's do not want to pay incorrectly, which results in more money lost may times. For instance, does your husband carry insurance on your entire family, but this year you started working and are receiving insurance via your employment? This is something insurance has to know to pay accurately and to determine which company is responsible for paying primary vs secondary vs tertiary. Having said all of that, assuming you went to the dr for a well visit, and you have not exceeded your yearly allowable on well visits (most plans pay for 1/yr) and you went to an In network doctor (b/c if they are out of network, and potentially a new practice, a W-9 might not be on file, which could either hold up the processing of the claim or the payment of the claim). And all information you have supplied to your dr is accurate, and its the not January 1 and you were only seen for a well visit, then it shouldn't take more than a week for it actually processes. Again, assuming the above is all in play. But, something else you have to consider is that just because you see the dr today, does not mean your claim will be submitted to them today. Also, once the insurance processes the claim, they don't send out EOB's on a daily basis (most don't at least, but there are some that do). Many do this once a week. So as you can see, while it seems like a very simple thing to do, in many cases, it's not that simple. SEVERAL situations play a part in how long it takes...like I didn't even get into the whole "system issue's" above...which can delay claims processing for very long periods of time, depending on the issue.

So they shoot their guns in the air and pack up their shit and go home…. the point being that war is a lot of waiting, hurry up-wait, hurry up-wait, etc (sound familiar???)I often run into this problem BC I am a “Gulf War Vet” (technically I am a post desert storm active operations southern watch and operation something else) BUT I was in h.s. When the actual war happened. But in 1995 (AFTER I graduated from h.s. and joined the military) I was sent in under a classified op (still) and people will question my status and medal- it drives me nuts BC to date the op is still classified that I served in and it was a nasty assignment to say the least…You went, you showed up, we “sign a blank check payable to the United States of America offering everything from our service to our lives” you didn’t duck out, hide in Canada, you went, I went (even though I have boobs) your a combat vet- don’t compare yourself to soldier Smith or Officer dipshit…. People seem to be convinced that ALL combat is like Platoon, Tigerland, or Full Metal Jacket and it isn’t…I met this AMAZING CPT a couple years ago, he was the sole survivor of a military aircraft disaster… he’d taken a space A flight to surprise his family (the plane crashed) and he had fingers and body parts that literally cremated while still attached to him… someone in the room asked him if he wished that he didn’t or couldn’t remember the crash; he said he was grateful to remember everything because it would be awful to not remember why and or what had made him the way he was…. BE GRATEFUL you can remember… I have 2–3 journals about being in a hostile fire zone and I picked one up years ago and it was so weird BC it was my writing BUT I had NO idea what I was talking about, it put me into the panic attack from hell. I had to try and find people I served with to fill in the blanks, they wrote letters that literally SHOCKED me. I still have NO recall of the majority of their letters. I found my “war book”, my crew checklists, not a single tiny sliver of recognition…my point is that we aren’t all (thank God) going to have to survive a plane crash, stab somebody with the bayonet attached to your gun, see “the whites of their eyes”, engage in hand to hand combat, take prisoners, etc. SOMETIMES we are just going to hurry up and wait and wait and wait and then pack our shit and go home… YOU SHOWED UP!!! You ARE a combat veteran, you’ve earned that title and that recognition- THANK YOU BROTHER!!!!And IGNORE the ignorant comments that discount you, your service, or your willingness to head to the front line.
