More than likely, poor Katie did injure herself during that fall, but it was to her cervical spine not to the arteries feeding her brain. She thus sought chiropractic treatment and therein started the disastrous cascade that led to her death. The year was 1986...I was was in my first year of my first job as an interventional radiologist. The San Francisco medical group I had joined was too small to subspecialize so I did a fair amount of neuroradiology as well. This included performing angiograms of the neck and brain. One night, a beautiful twenty-something year old woman came in with the signs of stroke. She was the picture of health (and, as the girlfriend of a very wealthy local financier she could afford any beauty and health treatments she desired). The working diagnosis by the team of physicians that amassed in her room was an ruptured aneurysm or AVM (arteriovenous malformation). The only problem was there was no blood in the brain on the CT scan...only signs of an infarct (stroke). Aneurysms and AVMs bleed when they rupture.I performed the angiogram (this was before we had CT or MRI angiography that now eliminate the need for this type of invasive procedure, by and large). She had a dissection of the carotid artery...a splitting of the layers such that the lumen (the inside of the tube) was choked off. Stroke occurs if the other arteries in the brain can't take up the slack fast enough (so called, collateral pathways) or blood clots form on the closed off artery and travel to the brain clogging off other arteries. This is what a dissection looks like:Sure enough, she had been to a chiropractor earlier that day. I was fascinated by the correlation but there was scant literature about this. Over the next 25 years of my career, I saw several similar cases. There have been scientific articles written about this (here: Chiropractic treatment of the neck can be a risk factor for stroke , for example). There are even lay articles: here's one from Forbes: Warning: Neck Adjustments Might Lead To Stroke. Ask any busy ER nurse or physician and they have likely seen a few cases.Yet they continue to occur and this is a tragedy (and malpractice). The carotids and vertebral arteries are particularly vulnerable (and certain inherited conditions make them even more likely to dissect with minimal trauma). All chiropractors should be aware of these risks yet we continue to see cases.I am not indicting the field of chiropractic, about which I know very little. I do know that "manipulations" for lower back pain have helped a number of patients and friends I've spoken with. Obviously there must be some efficacy or patients wouldn't return for these treatments. Although the possibility of the placebo effect cannot be excluded, it doesn't matter if the patient feels better, right?But there is no role for aggressive neck manipulation that puts these delicate arteries at risk (the risk is even higher in elderly patients with "hardening of the arteries" or atherosclerosis). Students in most medical schools often recite the Hippocratic oath, upon which Primum non nocere, "First do no harm" is loosely based. I can only hope that chiropractors subscribe to the same adage.
