It's very common to forget something when you have the doctor in front of you. This is my profession and even I do it. Heck, I hope it's filled out! ( If it was entered before my time, it might not be. Now, you can't see the doctor without it filled out.)It's so important my practice asks you to re do them every year. Patients hate it, complain about it, loud!
