hello everyone my name is esther i'm the pediatric clinic manager here at the hamilton county health department during this video we'll be walking you through the process on how to make a request for an immunization record for your child with school starting up again many parents find that they need an updated immunization record to turn into the school we'll be showing you how to do this in the video that follows in your internet browser search health dot hamilton the letters tn.org select current health department services icon select highlighted form under immunizations shot record once you have reached the form please be sure to fill out all the required information then hit submit please be sure to submit a separate form for each child you are making the record request for after you submit it we will get it here and we will start working on it in the order that we receive it we will give you um a call at the phone number you provide or we'll shoot you an email if we need any more
