The problem could be that on the Print dialog (i.e. the dialog that appears after you click File -> Print) there is a drop-down menu that determines what content will be printed. I believe this drop-down is labelled 'Comments and Forms'.Make sure that the selected item is 'Document and Markups', because if you have just 'Document' selected it will not print out the form field contents.
