There is no such form in US taxation.  Thus you can not fill it out.  If you mean a 1098 T you still do not.  The University issues it to you.  Please read the answers to the last 4 questions you posted about form 1080.  IT DOES NOT EXIST.

Trust me, I lived it, you are probably being scammed. I was selected for a summer internship 2016.I tried to be very open while filling the preference form: I choose many products as my favorite products and I said I'm open about the team I want to join.I even was very open in the  location and start date to get host matching interviews (I negotiated the start date in the interview until both me and my host were happy.) You could ask your recruiter to review your form (there are very cool and could help you a lot since they have a bigger experience).Do a search on the potential team.Before the interviews,  try to find smart question that you are going to ask for the potential host (do a search on the team  to find nice and deep questions to impress your host). Prepare well your resume.You are very likely not going to get algorithm/data structure questions like in the first round. It's going to be just some friendly chat if you are lucky. If your potential team is working on something like machine learning, expect that  they are going to ask you questions about machine learning, courses related to machine learning you have and relevant experience (projects, internship). Of course you have to study that before the interview. Take as long time as you need if you feel rusty. It takes some time to get ready for the host matching (it's less than the technical interview)  but it's worth it of course.

Computation of Retired Pay The formula for computing equivalent years of service for Reserve retired pay at age 60 is fairly simple: Total number of Creditable Retirement Points, divided by 360. The formula computes the number of equivalent years of service the soldier has completed (comparable to full-time service).

Military Retirement Pay Computations will receive, over their lifetime, $104 in retirement pay for everyday they served on active duty. Guard/Reserve Retirement is based on a “points' system. The average number of points received is 74/year. The Formula is Total Points Divided by 365.

There are three ways to compute reserve retirement, which is based on when you first entered military service (not just reserve service). If you have 20 qualifying years of service and your initial date of entry is: Before Sept. 8, 1980, divide the number of retirement points by 360.

Military retirees, those who receive disability or other benefits from the Department of Veterans Affairs, federal retirees, and social security recipients will see a 2.8 percent pay raise in their monthly checks in 2019.

Your retired pay will be deposited to your account on the first business day of the month following the end of the month. Your first payment for retired pay normally will arrive 30 days after your release from active duty, or, on the first business day of the month following the month of first entitlement to pay.
