What the Account Transfer Request Form Is

An Account Transfer Request Form is a standardized document used to move assets, cash, or securities from one financial account to another, or to transfer custody between institutions. It records account numbers, account owners, transfer types (transfer in kind, liquidation), and authorization from the account holder. Financial firms, broker-dealers, custodians, and clearing agents rely on this form to initiate, track, and reconcile transfers while meeting regulatory and anti-fraud controls. The form provides the paper—or electronic—trail needed to validate instructions and confirm completion with both sending and receiving parties.

Why Use an Account Transfer Request Form

The form reduces errors by capturing essential transfer details and signer authorization in a single record. It clarifies responsibilities between sending and receiving institutions, helps satisfy audit and compliance needs, and documents customer consent for movement of funds or securities.

Why Use an Account Transfer Request Form

Who Completes and Signs This Form

Account holders, financial advisors, broker-dealer operations staff, and custodial onboarding teams commonly complete or submit the form.

  • Individual investors: submit their own account numbers, provide ID information, and sign to authorize outbound transfers.
  • Advisors and RIAs: complete client-authorized sections and attach advisory agreements or written consent when required.
  • Custodial operations: verify signatures, confirm account matching, and coordinate settlement instructions between broker-dealers.

All signers must have authority to direct transfers; third-party delegation requires documented power of attorney or similar authorization.

Typical Signatories and Roles

Account Holder

The natural person or legal entity listed on the account; responsible for providing accurate account numbers, tax ID or SSN, and signing to authorize the transfer. Financial institutions typically require the account holder's signature to process the request and may perform identity verification before initiating movement of assets.

Authorized Agent

A named representative such as a financial advisor or someone holding a valid power of attorney; must present written authorization documents. Institutions will validate agent authority against account records and may require notarization or medallion signature guarantee depending on the asset type.

Essential Fields to Include

Account Owner: Full legal name
Account Number: Sending account ID
Receiving Account: Destination account ID
Asset Details: Securities or cash
Signature: Signed and dated
Contact Info: Phone and email

Common Pitfalls to Avoid

  • Providing an incorrect account number or CUSIP can delay settlement and may cause assets to route to the wrong destination.
  • Missing or mismatched names between accounts often triggers additional verification and can require corrective paperwork.
  • Failing to include required supporting documents, such as a Medallion Signature Guarantee or power of attorney, will pause processing.
  • Using abbreviated state names or incomplete addresses can prevent legal service and complicate institutional matching.

Step-by-Step: Completing the Account Transfer Request Form

Follow these steps in order to prepare a complete, institution-ready transfer request.

  • 01
Identify accounts: Collect sending and receiving account numbers and custodial names.
  • 02
Describe assets: List securities by ticker/CUSIP and cash amounts precisely.
  • 03
Attach authorizations: Include power of attorney or advisory agreements if applicable.
  • 04
Sign and date: Have authorized parties sign; notarize if requested.

How Transfers Are Processed Between Institutions

Transfers follow a coordinated sequence involving both sending and receiving parties and often a clearing agent.

  • Submission: Sender submits the completed request to the current custodian.
  • Validation: Custodian verifies ownership, signatures, and eligibility of assets.
  • Settlement: Assets move via ACH, DTC, or broker-to-broker settlement instructions.
  • Confirmation: Receiving institution posts assets and sends confirmation to both parties.

Core Sections to Build into a Professional Form

Design the form with discrete sections for identification, asset detail, authorization, and institution-specific routing to reduce processing back-and-forth.

Identification

Include full legal name, tax ID or SSN/EIN, date of birth for individuals, and complete mailing address for legal identification and anti-fraud checks.

Asset Inventory

Provide detailed security descriptions: ticker symbol, CUSIP, number of shares, account class, and whether to transfer in kind or liquidate to cash prior to transfer.

Authorization

Clear signature blocks with printed name, title for corporate accounts, date, and any notarization or medallion guarantee requirements specified by the institution.

Routing Instructions

Fields for sending custodian name, receiving custodian details, DTC number or wire instructions, and internal reference numbers to ensure accurate routing and reconciliation.

Accuracy and Compliance Best Practices

Adopt standard practices to lower processing time and reduce administrative rework.

Use complete legal identifiers
Always enter the exact legal name and tax identification number on the form. Mismatches between accounts often trigger manual review and require notarized affidavits to correct.
Attach required authorizing documents
Include powers of attorney, trust certification, or corporate resolution documents where an agent signs. Institutions will not accept agent instructions without supporting authority.
Specify transfer method
Declare whether the transfer is an in-kind transfer, full position transfer, partial transfer, or liquidation. Vague instructions can produce unexpected sales or partial moves.
Retain signed copies
Keep a dated, signed copy of the request and any confirmations. Retention supports dispute resolution and audit trails if settlement discrepancies arise.

Timing Expectations and Typical Deadlines

Processing time depends on asset type, required guarantees, and both institutions' clearing processes.

Cash transfers:

Typically 1–3 business days for ACH or wire settlements.

Securities transfers:

Often 3–7 business days via DTC, longer if manual handling required.

Medallion guarantee delays:

Obtaining a medallion signature can add multiple days based on provider availability.

Incomplete paperwork:

Missing information pauses processing until corrected; expect variable delays.

Institutional cutoffs:

Each firm sets its own submission cutoff times for same-day processing.

Optional but Recommended Clauses and Fields

Include a set of optional fields to handle exceptions, special routing, and third-party authorities efficiently.

Special Instructions

A free-text area to indicate partial transfers, tax lot selection (FIFO, specific ID), or restrictions such as 'do not liquidate' to avoid unintended sales.

Tax Withholding

Checkboxes and fields to indicate backup withholding status, foreign tax forms, or tax-exempt designations to ensure correct tax treatment on liquidation proceeds.

Medallion Guarantee

Flag whether a medallion signature guarantee is required; include an attachment area for the physical stamp or electronic equivalent.

Agent Authorization

Fields to capture advisor/broker details, CRD number if applicable, and an attestation that the agent is authorized to act on behalf of the account holder.

Error Correction

A procedure note describing how to submit amendments, contact points for corrections, and any required re‑execution of signatures.

Disclosure Acknowledgment

A short paragraph the signer initials to confirm awareness of fees, processing windows, and potential tax consequences related to the transfer.
