Terms, Warranties and Revocation

By signing this authorization, the undersigned certifies that the account information provided is true and correct and that the undersigned is an authorized signer on the referenced bank account. The undersigned authorizes the Payee to debit the designated account and to credit the account for any entries to correct erroneous debits.

This authorization is subject to the rules of the National Automated Clearing House Association (NACHA) and to any applicable state and federal laws. The Payee agrees to comply with applicable NACHA operating rules. The Customer agrees to hold the Payee harmless for dishonored or reversed transactions resulting from inaccurate information provided by the Customer or insufficient available funds.

Revocation: This authorization will remain in effect until the Payee receives written notice of revocation from the Customer at least 30 days prior to the next scheduled debit. Revocation does not affect debits previously initiated in accordance with this authorization.

Liability: The Customer is responsible for notifying the Payee promptly of any change in account information. The Customer agrees to indemnify and hold the Payee harmless from all claims, losses and expenses (including reasonable attorneys' fees) arising from a breach of this authorization or from failure to provide accurate account information.
