By signing below, the undersigned authorizes the Initiator named above to originate automated clearing house (ACH) credit or debit entries, as specified, to the payee bank account identified in this form and to debit or credit the Initiator's account as necessary to effect such transfers. The undersigned certifies that they are duly authorized to execute this ACH Payment Approval Form on behalf of the Initiator and that all information provided is true and correct.

The Initiator represents and warrants that: (a) the named payee is entitled to the funds described; (b) the account information supplied is accurate; (c) no payment will be made in violation of applicable law or contract; and (d) the Initiator will indemnify and hold harmless any bank, originating depository financial institution, or other party for losses, liabilities or costs arising from inaccurate information or unauthorized transactions resulting from this authorization.

This authorization is intended to comply with applicable ACH rules and the National Automated Clearing House Association (NACHA) operating rules. The Initiator acknowledges that ACH transactions are subject to the rules of the ACH network and agrees to be bound by such rules.
