What the Acord 130 Insurance Form Is

The Acord 130 Insurance Form is a standardized industry form used to document and transmit insurance policy data between agents, carriers, and third parties. It collects policy identifiers, effective dates, coverage limits, and producer information to create a consistent record for underwriting, claims handling, and verification. The form is not typically a contract in itself but supports policy administration, proof of insurance and internal workflows across insurance carriers and brokers.

Why the Acord 130 Matters for Coverage Management

Using the Acord 130 reduces ambiguity when sharing policy details and helps standardize communications among agents, carriers, and clients.

Why the Acord 130 Matters for Coverage Management

Who Typically Prepares and Receives This Form

The Acord 130 is used by insurance producers, carrier underwriting teams, and insureds for policy verification and administrative tasks.

  • Insurance producers and brokers — prepare and send completed forms to carriers and clients to document binding information and support renewals.
  • Carriers and underwriters — use the form to ingest standardized coverage data into policy systems and confirm limits and endorsements.
  • Policyholders and third parties — receive the form as proof of insurance for contracts, leases, or certificate requests.

Proper completion by the producer and timely distribution to all parties reduces processing delays and coverage disputes.

Common Roles That Sign or Authorize the Form

Producer / Agent

A licensed insurance producer completing the Acord 130 must ensure accuracy of policy numbers, effective dates, and limits; their signature or authorized electronic authorization confirms the provided data reflects carrier records and binds distribution responsibility.

Carrier Representative

An authorized carrier employee who validates policy information and may countersign or attach an acknowledgment; their acceptance indicates carrier confirmation of coverage details for third-party verification and claims intake.

Essential Elements Found on a Professional Acord 130

A correctly completed Acord 130 contains discrete fields for parties, policy specifics, coverage lines, and signature metadata to enable accurate carrier processing and external verification.

Producer Info

Broker name, license number, contact details and producer code used to route the form and link it to agency files and commission systems.

Named Insured

Exact legal name and mailing address of the insured; mismatches with policy records can trigger delays or rejection of requests.

Policy Identifiers

Carrier name, policy number and policy type fields used to match the form to a specific insurance contract and policy term.

Coverage Lines

Columns for limit, deductible, and status for each line of coverage (e.g., General Liability, Auto, Property), enabling quick assessment of protection scope.

Effective Dates

Policy inception and expiration dates in MM/DD/YYYY format determine coverage period and are critical for determining active coverage.

Signature Block

Area for producer or carrier signature, printed name and date; electronic signatures must provide intent, attribution and an audit trail.

Step-by-Step: Completing the Acord 130

Follow these steps to complete and circulate an accurate Acord 130 that carriers and third parties will accept.

  • 01
Gather documents: Collect current declarations and policy endorsements for reference.
  • 02
Enter identification: Populate insured and policy identifiers exactly as shown.
  • 03
Confirm coverage: Verify limits, deductibles and endorsements match the carrier record.
  • 04
Sign and send: Obtain authorized signature and distribute with audit trail.

Typical Digital Workflow Settings for Submission

Configure your digital workflow to ensure validation, signer authentication and a retained audit trail before sending the Acord 130.

Field Configuration
Authentication Email link or SMS code for signer verification
Required Fields Make producer, insured, policy number, dates mandatory
Audit Trail Enable IP, timestamp and signer attribution logging
File Format Use PDF/A or flattened PDF for archival consistency

How Electronic Submission Typically Works

Electronic submission follows a straightforward route: prepare, validate, authenticate, sign, and archive with a traceable audit record.

  • Prepare document: Upload a completed PDF or map fields from a template.
  • Add recipients: Specify producer, carrier contact, and any third-party viewers.
  • Authenticate signer: Choose email, SMS, or stronger verification methods.
  • Execute and store: Signer completes e-signature and system stores audit trail.

Platform and File Requirements for eSubmission

Verify your platform supports PDF and audit-trail export before starting a digital Acord 130 workflow.

  • File types: PDF, DOCX supported
  • Integrations: Works with CRM and document storage
  • Authentication: Email, SMS, or enterprise SSO

Use a platform that provides encryption in transit and at rest, an auditable certificate of completion, and basic signer authentication to meet carrier expectations.

Timing: When to Prepare and Send the Form

Timely delivery avoids coverage gaps and supports underwriting and claims workflows; the trigger depends on the use case.

New policy issuance:

Send concurrently with policy documents upon binding.

Renewal updates:

Provide at renewal effective date to confirm continuity.

Certificate requests:

Return within agreed service-level period or contract deadline.

Claims verification:

Supply immediately when carrier requests coverage proof.

Regulatory audits:

Produce promptly for examinations or audits.

Security and Compliance Considerations for Stored Forms

Encryption: TLS 1.2/1.3 in transit
At-rest: AES-256 encrypted storage
Certifications: SOC 2 Type II and ISO 27001
HIPAA: BAA required for PHI
Audit Trail: Detailed timestamp and IP logging
Access Controls: Role-based permissions and SSO

Risks and Consequences of Inaccurate Forms

Claim Delay: Possible delayed payment
Coverage Gap: Unintended lack of coverage
Contract Breach: Third-party contract noncompliance
Regulatory Action: State fines or notices
Data Exposure: PHI or PII mishandling
Reputational Harm: Loss of client trust

Common Preparation Errors to Avoid

  • Entering an incorrect policy number is a frequent error that prevents carriers from matching the form to the correct coverage record and causes processing delays.
  • Using informal or trade names instead of the insured's legal name can create ambiguity and may invalidate third-party reliance on the document.
  • Failing to record the correct effective or expiration dates can create apparent coverage gaps and lead to claim denials or disputes.
  • Neglecting to capture the authorized signer's name, title, and date reduces the form's probative value during audits or claims reviews.

Practical Tips for Accurate, Efficient Completion

Adopt consistent workflows and validation checks to reduce errors and speed distribution.

Validate policy data against carrier records
Before sending an Acord 130, cross-check policy numbers, named insured, and coverage limits with the carrier’s policy administration system to avoid mismatches that cause rework and delay verification.
Use standardized templates and required-field validation
Configure your digital template to require key fields—producer, policy number, effective dates and signature—to ensure forms are complete before submission and reduce back-and-forth with carriers.
Employ authenticated electronic signatures with audit trails
Use eSignature that captures signer attribution, timestamp, and IP to create admissible records of intent and simplify later audits or dispute resolution without manual notarization.
Archive copies with retention metadata
Store completed forms in a records system with retention tags and secure access controls so you can retrieve documents for claims, audits, or regulatory requests.
