What an Additional Information Request Is and When It’s Used

An Additional Information Request is a formal written request used to ask a party for clarification, missing documents, or further details needed to complete an application, review, claim, or transaction. Typical senders include employers, vendors, insurers, lenders, and government agencies. The document identifies the item(s) requested, the reason for the request, the deadline for a response, and any format or authentication requirements. It is not usually a contractual amendment but can become evidence supporting later decisions or filing actions if retained with the underlying file.

Why a Clear Additional Information Request Matters

A concise, well-structured request reduces delays, prevents misunderstandings, and creates a documented audit trail showing what was asked for and when. This supports compliance with regulatory timelines and reduces risk of disputes or processing errors.

Why a Clear Additional Information Request Matters

Typical Users and Recipients of an Additional Information Request

Recipients should respond promptly in the requested format and retain a copy of both the request and the response for the project record and compliance audits.

  • Human resources teams requesting identity or tax documentation during onboarding.
  • Underwriters and claims examiners requesting supporting documents for applications or claims.
  • Government agencies or contractors requesting supplemental documents for regulatory review.

Essential Parts of a Professional Additional Information Request

A properly composed request includes concise context, a precise list of required items, a clear deadline, delivery instructions, contact information, and any legal or privacy notices. Each element helps the recipient understand what is required and reduces back-and-forth.

Identifier

Unique file or request ID and reference to the original matter to ensure correct routing and association.

Purpose

One-line explanation of why the information is needed and how it will be used, to establish relevance and lawful basis.

Requested Items

A numbered list of documents or data points with precise descriptions to minimize ambiguity about acceptable formats.

Deadline

A specific date in MM/DD/YYYY format and any consequences for late or missing responses.

Delivery Instructions

Preferred submission channels, file formats (PDF, DOCX), and address or secure upload endpoint.

Privacy Notice

Statement about data protection, applicable legal bases (e.g., HIPAA or privacy law), and whether encryption or a BAA is required.
