Recitals and Purpose

This Adoption Agreement (this "Agreement") is entered into this day of , , by and between the parties identified above for the purpose of effecting the legal adoption of the child named herein.

1. Relinquishment and Consent. The Birth Parent(s) hereby knowingly and voluntarily consent to the termination of parental rights and to the adoption of the child by the Adopter(s). The consent is given freely, without undue influence, and with full understanding that, upon entry of a final adoption decree by a court of competent jurisdiction, parental rights will be terminated.

9. Indemnification.
