What Adult Client Paperwork Covers

Adult Client Paperwork is a standardized intake and consent packet used to record personal, legal, and administrative information for an adult receiving professional services. Typical content includes identifying details, contact and emergency contacts, insurance and billing data, informed consent, privacy disclosures, treatment or service agreements, and authorization to release records. When handled electronically, the packet must comply with ESIGN/UETA and any sector rules such as HIPAA for protected health information.
