What the Advisor Assist Template Is and When It’s Used

The Advisor Assist Template is a standardized, fillable document designed for advisors to collect client details, authorizations, disclosures, and signatures needed to initiate advisory services. It combines client identification, scope of engagement, fee disclosures, consent language, and signature blocks so the advisor and client have a single reproducible record. The template is format-neutral and intended for both paper and electronic workflows; when executed electronically it must meet ESIGN (15 U.S.C. ch. 96) and applicable state UETA/ESRA requirements to be enforceable.

Why a Consistent Advisor Assist Template Matters

Using a uniform Advisor Assist Template reduces ambiguity about responsibilities, preserves required disclosures, and creates an auditable record suitable for regulatory review. Consistency supports compliance with ESIGN and state e-signature frameworks while making each engagement easier to administer.

Why a Consistent Advisor Assist Template Matters

Who Typically Completes the Advisor Assist Template

The template is commonly completed by licensed advisors and their administrative staff before a client engagement begins.

  • Registered Investment Advisors and their client onboarding teams who need documented consent and fee disclosures.
  • Wealth managers and financial planners collecting KYC details, billing authorizations, and advisory agreements.
  • Compliance officers and legal teams verifying disclosures, records retention, and electronic consent procedures.

Departments across compliance, operations, and client service benefit from standardized fields and a clear signing sequence to reduce rework and support audits.

Key Roles Involved in Preparing and Signing

Registered Investment Advisor — Principal

Typically prepares the template, confirms suitability and disclosures, and signs on behalf of the advisory firm; responsible for maintaining the advisor-side record and ensuring e-sign procedures meet ESIGN and state law.

Client Signatory — Individual

Provides personal information, grants consent, and signs the document; must confirm identity and agree to receive records electronically when applicable under 15 U.S.C. ch. 96.

Core Elements to Include in a Professional Template

A complete Advisor Assist Template groups client identity, engagement terms, disclosures, consent language, and signature mechanics so it functions as a single source of truth for client onboarding and service delivery.

Client Identification

Full legal name, date of birth or business EIN, government ID reference, and contact information to support identity verification and tie the signature to a specific person or entity.

Scope of Services

A clear description of advisory services, deliverables, and exclusions so both parties understand obligations and performance expectations during the engagement.

Fees and Payment Terms

Precise fee structure, billing cadence, and any authorization for electronic payments or ACH debits to avoid disputes and support tax reporting where applicable.

Required Disclosures

Regulatory or firm disclosures such as conflicts of interest, privacy notices, and consumer-facing opt-in/opt-out language required by applicable law.

Consent and Authorization

Explicit consumer consent statement to receive records electronically and any ESIGN consumer disclosure if the template is used for consumer-facing transactions.

Signature and Audit Trail

Designated signature blocks, date fields, and an audit trail that records timestamps, IP addresses, and signer attribution for evidentiary support.

Step-by-Step: How to Complete the Advisor Assist Template

Follow a consistent sequence to minimize questions and make the signed record audit-ready.

  • 01
Prepare the document: Populate standard fields and attach any required exhibits.
  • 02
Verify identity: Confirm ID or use KBA/RON where applicable for remote signers.
  • 03
Add signature fields: Place signature, date, and initial fields for each party.
  • 04
Send for signature: Route in the correct order and capture audit trail data.

How to Configure an Online Workflow for the Template

Set up a reproducible workflow with defined fields, signer order, and storage destination to protect consistency and compliance.

Field Configuration
Signer Order Specify sequence: advisor then client
Authentication Email link or SMS code; KBA for higher assurance
Retention Location Designate secure cloud folder or ECM system
Notification Enable email copies and completion receipts

Digital Signing and Integration Considerations

Ensure the eSignature platform supports required authentication, audit trails, and integrations with your CRM or document repository.

  • Authentication Methods: Email link, SMS code, KBA, or advanced signer verification
  • Integrations: Connectors for Salesforce, NetSuite, Microsoft 365, Google Workspace
  • File Formats: Support for PDF, DOCX, and archival PDF/A output

Where to Send and Store Completed Templates

Route the signed template to systems that support search, retention, and audit reviews to maintain a defensible record.

  • Client Copy: Deliver a signed PDF to the client via email for their records.
  • CRM/Account Record: Store a copy in the client’s CRM file for operational continuity.
  • Secure Archive: Send to encrypted cloud storage or an ECM with access controls.
  • Compliance Folder: Retain an auditable copy with metadata for regulatory inspection.

Common Timing Rules and Filing Deadlines to Keep in Mind

Certain workflows connected to advisor templates intersect with tax and reporting deadlines; track deadlines separately for tax forms and regulatory filings.

W-9 / TIN Requests:

No fixed deadline; provide upon payer request to avoid backup withholding.

1099-NEC Filing:

Recipient and IRS deadline is January 31 each year.

1099-MISC IRS Filing:

Paper filing by Feb 28; electronic filing by Mar 31.

Individual Tax Return:

Form 1040 due April 15, extensions to October 15 possible.

Record Retention Start:

Retention typically measured from effective date or filing date.

Common Mistakes to Avoid When Preparing the Template

  • Entering informal or abbreviated legal names that do not match government IDs, causing identity verification failures or payment issues.
  • Failing to obtain explicit electronic consent for consumer-facing records, which can undermine ESIGN-compliant enforceability in disputes.
  • Omitting tax identifiers or using incorrect TINs, which can trigger backup withholding and IRS information return penalties.
  • Not preserving the audit trail or timestamps when signing electronically, reducing evidentiary value for regulatory review.

Key Penalties and Legal Risks from Errors

Information Return Penalties: IRC §6721 penalties for late/incorrect 1099s
Backup Withholding: 24% withholding for missing/incorrect TIN
I-9 Violations: Form I-9 paperwork fines per 8 CFR §274a.2
HIPAA Breach Liability: Civil penalties and corrective action obligations
Contract Enforceability: Invalid signatures can lead to voided agreements
Notarization Errors: Improper notarization may invalidate certain filings

Validate identity early
Confirm government ID, TIN/EIN, or use knowledge-based verification before sending for signature to reduce rework and verification delays.
Require explicit consent
Include a clear ESIGN consumer disclosure for consumer-facing transactions and capture affirmative consent in a timestamped field to support enforceability.
Preserve the audit trail
Store signed documents with their metadata, timestamps, and signer attribution in an access-controlled repository for compliance and dispute resolution.
