Given: to secure a certain Promissory Note in the amount of $ _______________ (Amount) , payable to Mortgagee. 2. On _______________________________ (Date, which must be within 30 days before the commencement of foreclosure proceedings), Affiant, as Mortgagee, mailed by certified mail, return receipt requested, to the said Mortgagor's last known address, a notice that the subject mortgage is in default and that said Mortgagee intended to foreclose it. The said Mortgagor has never responded to the said notice. 3. On ________________________________ (Date), Mortgagee made a personal inspection of the mortgaged premises and the inspection did not reveal that the Mortgagor or persons claiming under him were presently occupying or intending to occupy the mortgaged premises. Acknowledging that the foreclosure has been filed in the court of the county in which the mortgage has been sold, signed by the undersigned authority to have the record made. Annex b. Affidavit of foreclosure filed in the court of the county in which the mortgage has been sold, signed by the undersigned authority in and for said county and state. Annex c.
