Contract or Agreement for Dance Lessons This contract is hereby made this _____day of ___________________, 20_____ between _____________________________ (Client) and ____________________________ (Parent of Client) , who reside at _________________________________________________ __________________________________ ( street address, city, county, state, zip) , both of whom are hereinafter jointly and severally referred to as Client, and (Name of Teacher) , who resides at ____________________________________________________________________ ______________ ( street address, city, county, state, zip) , hereinafter referred to as Teacher . 1. Client hereby hires Teacher for __________________ (e.g., 5 one-hour) dance lessons at $_____________ per hour for the ________________________ (e.g., Spring 20____) Academic Semester. The date(s) and time(s) for the aforementioned lessons are listed below. If Client must cancel a lesson, 24 hour notice must be given to Teacher or the fee of $______________ will be immediately due and payable. A good faith effort will be made by Client and Teacher to reschedule a make-up appointment for the same week. Make-up sessions cannot be scheduled for dates beyond the current week. Should Teacher need to cancel a session, Client may reschedule for the same week. 3. If Client is not present at the pre-determined location when Teacher arrives or is more than fifteen (15) minutes late, he/she will still responsible for payment for the lesson. Until full payment is made, all future appointments will be suspended. Clients who arrive late will only be given a lesson for the remaining hour.
