- 1 – Copyright 2004 USLegalforms.com PARENT-CHILD CITIZENSHIP CONTRACT This Agreement (“Agreement”) is entered into as of this day of , 20 , by and between   , (hereinafter referred to as PARENT(s)) and , (hereinafter referred to as CHILD) who reside at in the state of for purposes of establishing and maintaining good citizenship habits. WHEREAS, CHILD acknowledges that there are certain kinds of people who can influence sa id CHILD in a negative manner. The way CHILD behaves and acts both at home and in public or school wi ll help to shape the rest of CHILD’S life. The people with whom CHILD associates will help making the right dec isions in life easier. That being a good citizen at all times and in any situation no matter how difficul t is the best way to live life. CHILD acknowledges the importance of a written agreement so that he/she knows what is expect ed of them and to know that there are always consequences for their actions or inactions. CHILD acknowledges that a contract is like a promise, and is not meant to be broken. WHEREAS, PARENT (S) agree that this Agreement is made out of love, respect, and concern for CHILD’S future well being. Now, THEREFORE, in consideration of the material promises set forth below the parties hereby agree as follows: 1. This Agreement is comprised of this document and shall incorporate any amendments, provided they are properly written and executed as provided herein. The terms of this Agreement shall control over any conflicting terms contained in ancillary documents. 2. This Agreement shall become effective when signed by both parties and shall conti nue in full force and effect until 20 . 3. HONESTY Honesty, truthfulness, and integrity should always be part of everything that you do. Being honest will help you lead a more successful and happier life. Honesty should be the cornerstone of your personality and it will be an identifying character trait in years to come. Initial _____ CLOTHS/APPEARANCE The way you dress and the way that you appear to other people says a lot about who you are . It is very important that you dress appropriately for the situation. We respect your right to dress the way you want and we trust that you will make the right choices regarding what to wear. As such, we will not make a list of what you can and cannot wear. It is not our intention to tell you what to look like. Initial _____ LANGUAGE The use of foul or abusive language is never acceptable behavior and will not be tolera ted. The way that you talk to your teachers, parents and friends, reflects upon you as a person. The use of cursive or deme aning language is never necessary. If ever you find yourself unable to know the difference between right and wrong- you can always come to us and we will talk about the situation. There is never a punishment for coming to your parents for advice. CHILD shall abide by the terms of the contract and shall not be given any privil eges until such compliance. THIS IS NOT NEGOTIABLE. 5. Privileges afforded to CHILD are at the discretion of PARENT (S) and shall be as follows: 7. This Agreement may be renegotiated at the following date 20 . Revision or renegotiation is based upon CHILD’S progress. CHILD will earn more or fewer privileges based on behavior demonstrated throughout the course of this Agreement. 8. Should CHILD choose to break the terms of this Agreement then the following sanctions will apply: a. First time infraction- b. Second time infraction- c. Third time infraction- AN INFRACTION MEANS FAILING TO ABIDE BY ONE OR ANY OF THE PROVISIONS OF THIS AGREEMENT. 9. PARENT (S) agrees to note any instances of good behavior and grant privileges accordingly. 10. PARENT (S) agrees not to choose or select friends for said CHILD but is acting only in the best interest of said CHILD and is looking out for the welfare of CHILD by prohibiting contact with the above named individuals or any persons of like mind and character. I have read, understand and consent to the terms of this Agreement and I agree to execute this contract to the best of my ability by abiding by all terms and conditions as stated.
