What the Anatomy Act Compliance Form Is

The Anatomy Act Compliance Form documents legally required consent and institutional acceptance for the handling, dissection, donation, transfer, or disposal of human remains or anatomical material. It records donor identity, authorized uses, transfer chain-of-custody, and any restrictions. The form serves as both a consent instrument and a compliance record for institutions such as medical schools, anatomy labs, pathology departments, funeral homes, and medical examiner offices. Where permitted, the form may be completed and retained electronically subject to ESIGN and state electronic transaction laws.

Why a Formal Compliance Form Matters

A standardized Anatomy Act Compliance Form creates a clear legal record of donor intent, institutional approval, and downstream obligations, reducing disputes, ensuring regulatory adherence, and improving traceability for ethical and legal review.

Why a Formal Compliance Form Matters

Who Typically Completes This Form

Professionals across clinical, academic, and regulatory settings complete or process these forms when accepting anatomical donations or transferring tissue.

  • Medical school administrators and anatomy lab coordinators managing donor records and educational use permissions.
  • Hospital legal or compliance officers reviewing institutional acceptance and consent language for regulatory alignment.
  • Funeral home or mortuary staff handling disposition authorizations and transfer documentation for families.

Roles vary by institution but usually include clinical staff, legal counsel, registrars, and authorized signatories for the receiving facility.
