Confidentiality

Recipient agrees not to disclose confidential records or donor information disclosed in confidence, except to veterinary providers or as required by law.

Release, Indemnity, and Medical Authorization

Recipient accepts the puppy in its present condition, subject to the disclosures above. Recipient releases and holds harmless Rescue Organization, its officers, volunteers, and agents from claims for incidents occurring after transfer of custody, except to the extent caused by Rescue Organization's gross negligence or willful misconduct. Recipient shall indemnify Rescue Organization for claims arising from Recipient's negligence.

In the event emergency veterinary care is required and Rescue Organization cannot be reached, Recipient authorizes veterinary treatment for the puppy and agrees to be financially responsible for emergency costs up to $ unless other arrangements are made in writing.

Governing Law and Entire Agreement

This Agreement is governed by the laws of the state of . Any legal action arising under this Agreement shall be brought in the appropriate state or federal court located in that jurisdiction.
