What the AORs Document Template Is

The AORs Document Template is a standardized Agent of Record appointment form used to transfer or confirm authority for an agent or broker to act on behalf of a policyholder or client. It identifies the parties, describes the scope of representation (lines of coverage or services), records effective and termination dates, and sets commission or fee arrangements. A properly completed AOR clarifies who may negotiate, receive commissions, and request policy changes. The template can be adapted for insurance, financial services, and corporate representation and may support electronic signatures under federal and state e-signature laws.

Why a Clear AOR Matters

A written AOR reduces disputes over representation, preserves commission and payment rights, and documents authority limits. It creates a verifiable record for compliance, audit trails, and consistent onboarding across carriers and administrative systems.

Why a Clear AOR Matters

Who Commonly Uses an AORs Document

Typical users include corporate policyholders, insurance carriers, brokers, and in-house legal counsel who need documented agent authority.

  • Insurance carriers and underwriters managing agent appointments and commission records.
  • Independent agents and brokers documenting exclusive or non-exclusive representation for clients.
  • Employers and benefits administrators assigning brokers for employee benefit plan interactions.

Use these roles to determine who must sign, how documents are routed, and what authentication is required.

Primary Signer Profiles

Agent

The appointed agent must be identified by full legal name, license number, and business address. Include authorized activities (e.g., bind coverage, request endorsements), territory limits, and any carrier exclusions to avoid scope disputes.

Policyholder

The policyholder or client provides the legal entity name, tax identification or SSN when required, and a primary contact. Their signature confirms explicit consent for appointment, effective dates, and how revocation must be handled.

Required Fields in the Template

Agent Name: Full legal name as shown on ID
Agent License: State and license number
Principal Name: Name of individual or legal entity
Policy Number: Insurer-issued policy identification number
Scope of Authority: Lines of coverage and permitted actions
Effective Date: Enter as MM/DD/YYYY format

Consequences of an Incorrect or Incomplete AOR

Contract Disputes: Ambiguous authority leads to litigation
Commission Disputes: Unclear appointment triggers payment disputes
Regulatory Penalties: State fines for improper licensing
Claims Denial: Carrier may deny claim adjustments
Tax Exposure: Incorrect TIN triggers backup withholding
Invalidated Appointment: Signature defects can void AOR

Common Preparation Mistakes to Avoid

  • Omitting the agent license number or state can delay carrier acceptance and create uncertainty about which individual or firm is authorized to act.
  • Using vague language such as 'handle claims' without defining limits often leads to disputes over whether the agent could bind coverage or make premium changes.
  • Failing to include a clear revocation method or to file a revocation promptly may result in continued commission payments to a former agent.
  • Submitting unsigned templates, inconsistent party names, or incorrect dates increases administrative rejections and can undermine electronic signature evidence under ESIGN.
