Complete 3b, if after an initial name availability check through the Corporate Database on our website or by phoning our office, the original name is unavailable for use in Rhode Island. A Fictitious Business Name Statement, Form 624A, must be filed and submitted with this application. The Fictitious Business Name Statement has a $50 filing fee. 4.
