What the Application for Utility Connection Is

The Application for Utility Connection is a standardized form used to request initiation or transfer of utility services (electricity, water, gas, sewer, or telecommunications) at a specific service address. It collects identifying information about the applicant and service location, authorizations to access meters or property, requested start date, billing account details, and any required deposit or credit-check consent. Utilities use the completed application to establish customer accounts, schedule service activation, and verify identity and residency before providing service.

Why a Complete Application Matters

A correctly completed Application for Utility Connection reduces delays, avoids service refusals, and establishes clear billing responsibility. Accurate information supports faster identity verification, correct meter assignment, and timely scheduling of field crews for connection or transfer.

Why a Complete Application Matters

Who Typically Completes This Application

The form is used by residential customers, commercial tenants, property managers, and developers to request new or transferred utility service.

  • Residential applicants: individuals requesting service at a primary or secondary residence.
  • Commercial applicants: businesses applying for utilities at a commercial address or single-meter multi-tenant building.
  • Authorized agents: property managers or contractors acting on behalf of an owner with written authorization.

Different signatories and required attachments depend on the account type (residential vs commercial) and whether the applicant is the property owner, tenant, or authorized agent.
