What the Appraisal Acknowledgement Form Is

The Appraisal Acknowledgement Form documents that an appraiser or property owner has provided, received, or reviewed an appraisal report related to a real property transaction or valuation engagement. It records the parties involved, the appraisal date, scope of work, appraisal purpose, and any limiting conditions or disclosures. The form creates a clear administrative record for lenders, insurers, servicers, tax authorities, and internal compliance teams, and supports auditability for underwriting and regulatory review. It is used alongside appraisal reports, contracts, and closing documents to establish provenance and acknowledgment of the valuation.

Why an Appraisal Acknowledgement Form Matters

An Appraisal Acknowledgement Form clarifies who received or reviewed the appraisal, helps prevent disputes, and creates a reproducible record for compliance and audit purposes. For regulated transactions, electronic acknowledgements are legally binding under the ESIGN Act (15 U.S.C. ch. 96) and UETA where adopted, provided intent, consent, attribution, and retention requirements are met.

Why an Appraisal Acknowledgement Form Matters

Who Typically Completes This Form

The form is commonly completed by participants in valuation workflows who need a formal record of appraisal delivery or receipt.

  • Lenders and loan processors – confirm appraisal delivery and acceptance during underwriting and closing.
  • Appraisers and appraisal firms – document delivery, scope, and any distribution restrictions.
  • Title and closing agents – record receipt of appraisal documents for closing packages.

Use the form to create a consistent trail among parties and reduce administrative gaps in real estate and collateral valuation processes.

  • Omitting delivery method or tracking details, which weakens proof of receipt and can delay underwriting or closing.
  • Allowing unsigned or improperly authenticated electronic signatures when the transaction requires higher-assurance signer verification.
