What the Approval Mockups Form Is and why it matters

An Approval Mockups Form documents stakeholder review and formal sign-off for design deliverables, capturing version, scope, requested changes, approver identity, and the final decision in one record. It typically includes project identifiers, mockup images or links, a clear approval choice (approve, approve with changes, reject), a dated signature block, and optional comments. Standardizing this form reduces ambiguity, prevents repeated revisions, and preserves an auditable history of decisions. For electronic execution, follow the ESIGN Act (15 U.S.C. §7001) and applicable state UETA rules to ensure enforceability and reproducible records.

Why using a standard Approval Mockups Form improves outcomes

A consistent form defines the scope of approval, names authorized signers, and creates a single source of truth for designers and stakeholders. It shortens review cycles, reduces rework, preserves evidence of consent, and supports downstream processes such as release planning and change control.

Why using a standard Approval Mockups Form improves outcomes

Who typically completes or signs this form

Typical users who complete or review this form include designers, product managers, marketing leads, and legal or compliance reviewers responsible for final approvals.

  • Design and UX teams — validate visuals, accessibility, and brand alignment before releasing assets.
  • Product managers and PMOs — confirm feature scope, acceptance criteria, and release timing tied to mockup approval.
  • Legal, compliance, or brand guardians — ensure required disclaimers, IP clauses, or regulatory controls are in place.

The list of approvers drives routing rules, who receives copies, and how retention is managed after sign-off.

Core elements every professional Approval Mockups Form should include

A complete form balances clear identification, precise versioning, reviewer intent, and an auditable signature mechanism to minimize downstream disputes and support recordkeeping.

Project Info

Project name, client or product code, requestor name, and contact details so reviewers can confirm context and find related materials.

Versioning

Mockup version number and date plus links or embedded images so approvers know exactly which deliverable is under review.

Scope & Notes

Explicit summary of what is being approved and any exclusions or constraints, preventing scope creep or ambiguous acceptance.

Decision Options

Predefined choices (Approve, Approve with Changes, Reject) with space for conditional notes to speed and standardize responses.

Signature Block

Name, role, signature, and date field for each approver; include printed name and contact for verification if needed.

Audit Trail

Timestamped activity log capturing who viewed, who signed, IP or device metadata, and any changes made to preserve evidentiary integrity.

Step-by-step: how to complete and obtain an approval

Follow these steps to prepare the mockup, route it for review, capture approvals, and store the completed record.

  • 01

  • 03
Route to Reviewers: Assign reviewers in order or parallel based on authority and role.
  • 04

  • Review: Designers and stakeholders examine mockups and comment.
  • Approve: Authorized signers record their decision and sign.
  • Archive: Store signed form and audit trail in project records.

Technical requirements for digital distribution and signing

Confirm your platform supports required file formats, signer authentication, and secure storage before sending approval requests.

  • File Formats: PDF, DOCX, PNG, Figma links
  • Authentication: Email, SMS, KBA options
  • Integrations: Salesforce, NetSuite, Google Workspace

Typical timing expectations and review deadlines

Set explicit deadlines and escalation rules to avoid indefinite reviews; document the review window on the form itself so everyone shares the same expectations.

Initial Review Window:

48–72 hours is common for a first-round response.

Revision Turnaround:

3–5 business days depending on scope of changes.

Final Sign-off Deadline:

Specify a date to prevent delays to release schedules.

Escalation Trigger:

Auto-reminder after 48 hours; escalate after 5 business days.

Retention Action:

Archive completed form within project repository immediately.

Common mistakes to avoid when preparing an Approval Mockups Form

  • Missing or inconsistent version labels that cause multiple parties to believe they reviewed different deliverables.
  • Vague approval scope such as 'looks good' without specifying screens, states, or assets covered by the approval.
  • Incorrect approver list that omits an authority or includes someone without signing rights, delaying release.
  • Relying on informal confirmations (chat or email) without a single signed form and audit trail documenting the decision.

Risks and consequences of an incomplete or incorrect approval form

Rework Costs: Increased hours and budget
Release Delays: Missed deadlines
Legal Exposure: IP or contract disputes
Compliance Gaps: Privacy or regulatory risk
Audit Failures: Missing verifiable trail
Customer Impact: Confusing or defective deliverables
