What the Aquamation Request Form Is

The Aquamation Request Form documents authorization for an alkaline hydrolysis disposition (commonly called aquamation or water-based cremation). It records the decedent's identifying details, legal next of kin or authorized agent, disposition instructions, and any required permits or medical releases. Funeral providers and healthcare facilities use the form to confirm legal authority, verify identity, collect signatures, and schedule the aquamation service while meeting local regulatory and public health requirements.

Why a Clear Request Form Matters

A well‑prepared Aquamation Request Form provides legal authorization, documents consent, reduces processing delays, and creates a permanent record of the decedent's wishes and the authorized signer. Accurate forms help providers comply with public health rules and expedite disposition.

Why a Clear Request Form Matters

Who typically completes the form

Organizations and individuals involved in disposition complete or submit this form.

  • Next of kin or legal representative — Family members or court-appointed agents who hold legal authority to direct disposition.
  • Funeral director or licensed provider — Completes logistics, verifies documentation, and files permits with local authorities.
  • Medical facility or coroner representative — Supplies medical releases or cause-of-death documentation when required.

Each signer should confirm their legal authority and provide accurate contact and identification information.

Common mistakes that delay processing

  • Incomplete identification fields — omitting a middle name or incorrect DOB often requires provider follow-up and slows scheduling.
  • Missing medical or coroner release — failure to attach required releases prevents issuance of disposition permits and stops processing.
  • Unsigned or improperly dated signatures — unsigned forms or inconsistent dates create legal uncertainty and may invalidate authorization.
  • Using incorrect signer — accepting signature from someone without legal authority can lead to disputes or regulatory noncompliance.

Risks and consequences of incorrect forms

Invalid Authorization: May prevent lawful disposition
Regulatory Hold: Service may be delayed until resolved
Legal Liability: Estate disputes or civil claims possible
HIPAA Violation: Improper PHI handling risks penalties
Coroner Rejection: Missing release can block disposition
Financial Loss: Additional storage, re‑submission, or legal fees
