What the AS9100 Certification Form Is and when it’s used

The AS9100 Certification Form documents an organization’s compliance with the AS9100 aerospace quality management requirements and summarizes the scope of a certified Quality Management System (QMS). It is used by suppliers, manufacturers, and subcontractors to request initial certification, notify a registrar of scope changes, or support surveillance and recertification audits. Typical entries include legal entity details, facility locations, processes in scope, key quality contacts, and references to related procedures and records that auditors and contracting authorities review.

Why a formal AS9100 Certification Form matters for aerospace suppliers

A completed, accurate form clarifies certification scope, streamlines audits, and documents evidence required by registrars and prime contractors. It reduces ambiguity in supplier qualification, supports contractual compliance, and makes surveillance and recertification cycles more predictable.

Who typically prepares and reviews this form

Final review is usually by an authorized signatory responsible for corporate commitments and for maintaining audit evidence.

  • Quality Manager responsible for QMS documentation and audit responses.
  • Compliance Officer or Regulatory Affairs who verifies contractual and regulatory alignment.
  • Operations or Plant Manager who confirms process scope and facility details.
