What the Attestation QED Form Is and when it is used

The Attestation QED Form is a structured affidavit-style document used to record a declarant's statement or confirmation about specific facts, conditions, or compliance items. Commonly used where a formal, signed declaration is required—for example, regulatory attestations, eligibility confirmations, or project closeout statements—the form captures identifying data, the attestation text, signature, and date. The format supports both paper and electronic execution and is often paired with supporting documentation such as invoices, certifications, or identity proof. Accuracy and clear signatory authority are essential for legal and administrative acceptance.
