What an Auditor Confirmation Form Is and When It’s Used

An Auditor Confirmation Form is a standardized request sent by an independent auditor to a third party (for example, a bank, vendor, or legal counsel) asking for written verification of account balances, transactions, obligations, or other information used in an audit. The form documents the auditor’s request and the third party’s response and becomes part of the audit evidence file. Confirmations support assertions about completeness, existence, rights and obligations, and valuation when corroborating client-provided records and are commonly used in financial statement audits, compliance reviews, and due diligence engagements.

Why the Auditor Confirmation Form Matters for Reliable Audit Evidence

A clear confirmation form produces independent, documented evidence about third-party balances or facts that reduces audit risk and supports the auditor’s opinion. Properly completed confirmations help demonstrate professional skepticism and satisfy auditing standards for external corroboration.

Why the Auditor Confirmation Form Matters for Reliable Audit Evidence

Who Typically Prepares and Responds to Auditor Confirmations

Typical preparers and recipients include audit firms, client finance teams, banks, vendors, and legal counsel.

  • External audit teams — send confirmations to verify balances, leases, guarantees, and contingent liabilities.
  • Corporate finance departments — gather and coordinate third-party responses and supporting documentation.
  • Banks and custodians — confirm cash, loan balances, collateral arrangements, and securities holdings.

Stepwise Procedure to Prepare and Send a Confirmation

Follow a consistent process from selection to receipt: prepare, send, authenticate, and document responses.

  • 01
Select Items: Identify accounts or obligations requiring external confirmation.
  • 02
Draft Form: Complete all fields accurately and attach client authorization if required.
  • 03
Send and Authenticate: Deliver via secure channel and use signer authentication where necessary.
  • 04
Record Response: Attach replies to workpapers and note any exceptions or non-responses.

Where to Send Confirmations and Typical Routing

Choose delivery channels that preserve independence and provide verifiable audit trails.

  • Direct to Respondent: Send confirmation only to the third party’s official contact address or secure portal.
  • Auditor Email/Portal: Use the auditor’s secure email or eSignature workflow to receive replies directly.
  • Client Coordination: Client may supply contact details but must not intercept or forward responses.
  • Retention: Store returned confirmations in audit workpapers with timestamps and metadata.

Configuring an Online Confirmation Workflow

Map each setting to authentication, delivery, and audit-trail requirements before sending.

Field Configuration
Authentication Email link, SMS code, or stronger KBA
Delivery Method Secure email or dedicated signing link
Response Routing Return to auditor address only
Attach Evidence Enable automatic attachment of signed PDF and audit log

Technical Considerations for Electronic Confirmations

Choose tools that provide authentication options, tamper-evident PDFs, and full audit trails.

  • Authentication Options: Email verification, SMS code, KBA, or SSO
  • Document Formats: PDF and DOCX with embedded audit metadata
  • Integrations: Connectors for storage and ERP systems

Essential Elements to Include in a Professional Auditor Confirmation Form

A well-constructed form balances clarity for the respondent with the auditor’s need for precise, verifiable information. Include contact details, reference data, and an unambiguous request.

Identification

Full auditee name, third-party name, branch, and account or reference numbers for precise lookup.

Purpose Statement

Clear explanation of why the information is requested and how it will be used in the audit.

Specific Query

Exact balance or item description and the date or period covered by the request.

Response Method

Specify return address, secure portal, or direct-to-auditor delivery instructions.

Authentication

State required authentication method (email, SMS, KBA) and any notary requirement.

Signature Block

Space for respondent name, title, signature, date, and contact information for verification follow-up.

Security and Compliance Controls to Preserve Confirmation Integrity

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: IP, timestamp, and action log for each signing event
Certifications: SOC 2 Type II, ISO 27001 available
HIPAA: Compliant — BAA required when PHI is present
ESIGN / UETA: Electronic signatures valid under federal and most state law
21 CFR Part 11: Support for FDA-regulated electronic records where required

Risks and Potential Consequences of Incomplete or Incorrect Confirmations

Qualified Opinion: Could result if material evidence is not obtained
Audit Delay: Non-responses prolong fieldwork and reporting
Regulatory Scrutiny: Regulators may investigate misleading or missing confirmations
Contract Risk: Incorrect confirmations can affect covenants or loan terms
Reputational Harm: Repeated issues damage auditor-client confidence
Legal Exposure: Potential litigation if material facts are misrepresented

Common Mistakes to Avoid When Preparing a Confirmation

  • Using informal or ambiguous account references that impede respondent lookup.
  • Sending confirmations to client-controlled addresses instead of the third party.
  • Omitting the exact date or currency for balance requests.
  • Failing to request direct return to the auditor or a secure channel.

Practical Tips for Accurate and Efficient Confirmations

Adopt standard templates and consistent routing rules to reduce exceptions and follow-up cycles.

Standardize Templates
Use a firm-approved template with required fields to ensure consistency and reduce respondent confusion.
Allow Adequate Lead Time
Set reasonable response windows (10–21 business days) and follow up promptly on non-responses.
Secure Channels
Deliver and receive confirmations via secure email, portal, or authenticated eSignature to protect integrity.
Document Follow-Up
Log all follow-ups and alternative procedures performed in case of non-response.

Timing and Expected Turnaround for Confirmation Workflows

Set realistic deadlines and monitor status to prevent audit schedule slips.

Request Lead Time:

Allow 10–21 business days for initial response depending on respondent type.

First Follow-Up:

Send a polite reminder after 7–10 business days if no reply.

Second Follow-Up:

Escalate to a supervisor or alternate contact after 14–21 business days.

Alternative Procedures:

Start alternative audit procedures if no response after two follow-ups.

Workpaper Completion:

Attach confirmations and finalize workpapers before issuing the audit report.
