What the Background Check Livescan Form Is and when it’s used

The Background Check Livescan Form records applicant biographic data and authorizes capture of fingerprints electronically for criminal history checks. It is completed by an applicant or authorized representative to initiate a state or federal background check using Livescan hardware and vendor software. The form typically includes identifying information, reason for the check (employment, licensing, volunteer), agency routing codes, and consent language required by the receiving agency. Use of Livescan speeds fingerprint capture and reduces transmission errors compared with ink-and-roll fingerprint cards.

Why the Background Check Livescan Form matters for compliance and accuracy

A correct Livescan form ensures fingerprints are routed to the intended agency and linked to the proper applicant record; errors can delay hiring, licensing, or clearance decisions. Properly completed forms reduce re-submissions and support traceable audit records for employment or regulatory compliance.
