What the Banking Account Details Form Is and When it’s Used

The Banking Account Details Form records bank account information needed for payments, direct deposits, ACH transfers, or vendor setup. It typically collects account holder name, bank routing number, account number, account type, and authorization language permitting debit or credit activity.
