What the BCBA Attestation Form Is and When It’s Used

A BCBA Attestation Form is a written declaration completed by a Board Certified Behavior Analyst to confirm professional credentials, supervision activities, service dates, or compliance with behavioral‑health program requirements. Typical uses include insurer or Medicaid documentation, employer credentialing, school district records, and provider audits. The form names the BCBA, lists certification or license numbers, summarizes supervised services or training provided, and contains a signed attestation that the information is true and accurate for billing, credentialing, or regulatory review.

Why a Clear Attestation Form Matters

A properly completed BCBA Attestation Form supports accurate billing, reduces audit risk, documents professional oversight, and speeds credentialing. It provides objective evidence of services and supervision that payers, employers, and regulators commonly require.

Why a Clear Attestation Form Matters

Who typically completes and relies on this form

Common signers and recipients include certified BCBAs, supervising clinicians, billing departments, and payer credentialing teams.

  • BCBA supervisors: Complete attestations to verify supervision hours, clinical oversight, and credential status for employee files and licensure.
  • Providers and billing teams: Use attestations to support claims to Medicaid and private insurers that require practitioner verification.
  • School districts and agencies: Retain attestations for special education records, service documentation, and local compliance reviews.

Knowing the audience helps ensure the form contains required identifiers, certification data, and the appropriate attestation language.

Core elements to include in a professional attestation

A complete BCBA Attestation Form organizes identity, credential verification, service scope, timeframe, client identifiers, and a signed attestation to support downstream claims, audits, or credentialing reviews.

BCBA Identity

Full legal name and contact details of the certifying BCBA; include employer and business address to connect the attestation to the provider.

Certification Number

Board certification or license number and issuing body with the certificate expiration date to allow payer and credentialing verification.

Scope of Services

Clear summary of supervision, assessment, or treatment activities being attested to, including service type and intervention focus.

Service Period

Start and end dates for the period being attested; use MM/DD/YYYY format to avoid ambiguity for audits and billing.

Client Details

Client name or unique identifier and, when required, date of birth or patient ID to reliably tie the attestation to specific services.

Signature Block

Signed name, printed name, professional title, and signature date; include witness or notary fields if the receiving organization requires them.

Step-by-step process to complete and deliver the attestation

Follow a consistent sequence to collect information, confirm identity, and preserve a verifiable record.

  • 01
Gather documents: Collect license, certification, client IDs, and service notes.
  • 02
Fill fields: Enter required data using specified formats and identifiers.
  • 03
Confirm identity: Authenticate signer via ID, credential, or approved eID method.
  • 04
Sign and send: Apply signature, date the form, and route to payer or records.

Typical electronic submission flow for attestations

Electronic workflows create an auditable sequence from upload to final storage; ensure authentication and retention are handled at each step.

  • Upload document: Add the attestation PDF or DOCX to the eSignature platform.
  • Place fields: Insert signature, date, and text fields where needed.
  • Select signer: Assign BCBA as signer and choose authentication strength.
  • Complete and store: Send signed copy to recipients and archive audit trail.

Technical considerations and compatible systems

Verify file formats, authentication options, and integrations with EHR or billing systems before eSubmission.

  • File formats: PDF, DOCX, and secure HTML supported
  • Authentication: Email, SMS code, KBA, or stronger
  • Integrations: Connects to EHRs, CRMs, and cloud storage

Security and compliance features to check

Encryption: TLS 1.2/1.3 in transit
Data at rest: AES-256 encrypted storage
HIPAA support: Business Associate Agreement available
Audit trail: Detailed timestamps and IP logs
Access control: Role-based permissions
Multi-factor: Two-factor signer authentication

Common preparation mistakes to avoid

  • Using an informal or partial name that doesn't match licensure records, which can block credentialing and payer verification.
  • Omitting precise service dates or using vague ranges that make it impossible to match billed sessions to the attestation.
  • Failing to include a certification or license number, delaying validation and possibly causing claim denials or audits.
  • Relying on an unsigned or improperly authenticated electronic signature without evidence of intent and attribution for legal acceptability.

Consequences of incorrect or missing attestations

Claim denials: Lost reimbursement
Credential delays: Licensing or credentialing hold-ups
Audit exposure: Potential recoupment or fines
Professional risk: Disciplinary review possible
HIPAA violations: Privacy breach penalties
Invalid affidavits: Form rejected by payer or agency
