Frequent errors that slow BCBS claim processing

  • Mismatched member name or ID that prevents eligibility matching and triggers manual review, often causing multi-week delays.
  • Missing or unclear procedure/diagnosis codes that lead to requests for additional documentation and potential coding edits.
  • Failure to attach required receipts, superbills, or referral authorizations, which results in immediate partial or full denial.
  • Using incorrect provider NPI or billing TIN that diverts payment or requires reprocessing and corrected claim submissions.

Consequences and administrative risks of incorrect claim forms

Claim denial: Loss of reimbursement
Delayed payment: Longer AR cycles
Audit exposure: Documentation requests
Recoupment: Repayment obligations
Potential fines: Regulatory penalties possible
Appeal costs: Additional administrative expense
