Waiver, Assumption of Risk and Release

I, the undersigned Client, acknowledge that I have disclosed all medical history, allergies, medications, and relevant conditions to the Service Provider. I understand that aesthetic procedures carry potential risks including, but not limited to, allergic reaction, infection, scarring, pigmentary changes, burns, or unsatisfactory aesthetic result. I knowingly and voluntarily assume all such risks associated with the services provided.

In consideration of the services rendered, I forever release, waive and discharge the Service Provider, its employees and agents from any and all claims, demands, liabilities, actions and causes of action, whether known or unknown, arising out of or relating to the services performed, except for gross negligence or willful misconduct.

I consent to the taking of photographs or video during the provision of services for recordkeeping, treatment progress and promotional use. I understand that my identity will not be disclosed without additional written consent.
